Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

Job overview
Opportunities have arisen for a Clinical Sister / Charge Nurse to join our innovative multi-disciplinary team. You must be an RGN or Paramedic with evidence of post registration practice at Band 5. Experience of multi-disciplinary / multi agency working is essential. You will be part of a team and have responsibility for an urgent caseload triage and signposting to Community Services, working initially under the direction of senior nurses within the Rapid Response Team. You will be working with Adult Services to develop and support this client group to remain in their homes with enhanced services or access a bed-based service to avoid avoidable hospital admissions. You will need to work flexibly to meet the demands of the service and be able to forge strong links with partnership agencies at Social Services and Acute Trust.

Main duties of the job
Under the direction of the UCR Service Manager you must assess a referral plan, implement, considering own clinical confidence and competence. You must implement enhanced programmes of care within our community provisions.
To be part of the CNC service to provide high standards of clinical judgement and intervention and decision making, and to triage nursing support for service users accessing the range of Community services
To co-ordinate enhanced clinical care programmes that are of the highest quality & standards and in line with the Walsall Healthcare NHS Trust’s policies and procedures
To work within a multi-agency / multi-disciplinary team adopting a ‘no blame’ culture
To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience, ensuring the highest standard.
Have a good sound knowledge of Community Services.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.

Detailed job description and main responsibilities
To be responsible for the triage / assessment of care needs and the
development, of programmes of care, within Scope of Professional Practice.
Demonstrate high level of judgemental/analytical skills when interpreting and performing patient assessments and care planning via the telephone and face to face.
Maintain accurate and up to date nursing records, maintaining patient
confidentiality, and statistical records as required by the service.
Work with other professionals to promote multidisciplinary / multiagency working.
Gain access to enhanced programmes of care in accordance with Walsall
Healthcare NHS Trusts integrated care pathways

Qualifications
Essential criteria
- Registered Nurse level 1 / 2
- Education to diploma level or able to demonstrate evidence of post registration experience at Band 5 within a similar primary care setting
Experience

- Post registration practice within a secondary or primary care setting – Demonstrate significant or Extensive experience
Knowledge
Essential criteria
- Basic working or in depth knowledge of NHS polices affecting hospital avoidance.
