Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

Walsall Healthcare NHS Trust are seeking to appoint an Elderly Care Consultant in Geriatric Medicine to support the acute elderly services. The posts will be working for 10 Programmed Activities (PA) per week with an additional PA available (subject to negotiation). The postholders will join a team of Elderly Care consultants to provide high quality patient care and support surgical patients who are undergoing emergency laparotomy. The postholder will have the opportunity to lead the development of POP services and improve the quality of care for patients presenting with emergency laparotomy surgery. 

The postholder will participate in a 1 in 18 on-call consultant rota for General Medicine during weekdays and weekends (1 PA per week will be remunerated). The weekend commitment includes morning and evening ward rounds in conjunction with an AMU Consultant covering all patients admitted to the Acute Medical Unit (AMU). There will be a requirement for 1 in 4 Saturday ward cover from 09:00 – 13:00. there is a requirement to review surgical patients on demand (0.75 PA) for this 1 in 4 Saturdays. In addition, there is a negotiable option of 1 in 6 weekend ward cover on Ward 29 09:00 – 15:00. (short stay ward) to cover do ward round on Saturday and Sunday.

Main duties of the job

The main duties are of the role will include the following:

  1. To have a responsibility to provide a high-quality clinical care to patients in Elderly Care and General Internal Medicine services and to continuously update practice based on evidence-based medicine and audit.
  2. To provide advice to colleagues in other specialities on elderly patients with problems (surgery).
  3. To support teaching and training of nurses, junior medical staff, medical students and to contribute to postgraduate medical education in general.
  4. To undertake educational/clinical supervisor duties for junior doctors
  5. To support and encourage audit and research
  6. To actively participate in CPD and personal development plans.
  7. To lead on the POP service

The post holder will be supported by an appropriately skilled medical secretariat.

Working for our organisation

There are 271 beds in Integrated Medicine provided at Walsall Manor Hospital including a 5 bedded Coronary Care Unit, a 46 bedded Acute Medical Unit for all emergency medical admissions and 13 critical care beds. There are purpose-built Endoscopy and coronary angiography units providing inpatient and outpatient diagnostic and therapeutic services. Outpatient Service facilities are available within the Department of Acute Medicine and are provided from both the main Outpatients Department together with specialist services within Clinical Measurement and Day Hospital, provided from a newly built Diagnostic Treatment Centre.

Walsall Healthcare is an integrated organisation aiming to meet patient needs by delivering services at the place of the patient’s choice and with the provision of acute admission facilities. One aspect of this post will be to lead the Frailty and FES service both in hospital and the community. Patients will currently be based on AMU/UECC moving to the Frailty Unit once it is fully established. This new post has been established to support A&E/Medical Admissions and the Frail Elderly Pathway to assess the frail elderly patient presenting to the Manor Hospital. The Elderly Care department have a robust programme for mentoring new consultant appointments and support study leave for CPD requirements.

Detailed job description and main responsibilities

A job plan and work schedule will set out agreed arrangements for how work is organised, where it is located, what in general terms the work comprises and when it is to be performed. The job plans are the same when undertaking duties, however the job plan may vary dependent of service needs. Job Plans will be discussed with the Clinical Director on appointment.

“Direct clinical care” (DCC) (work relating directly to assessment, prevention, diagnosis or treatment of illness) Includes outpatient and inpatient activities, multi-disciplinary meetings about and administration directly related to the above. “Supporting professional activities” (SPA) (that underpin direct clinical care) include participation in training, medical education, continuous professional development, formal teaching, audit, MDT clinical management and local clinical governance activities.

The working week for a full-time Elderly Care consultant is comprised of 10 PAs, each of which has a nominal timetable value of 4 hours. A consultant will spend 7.5 PAs on Direct Clinical Care with 2.5 PAs on supporting Professional Activities and clinical and educational roles for juniors.

Qualifications

Essential criteria
  • Basic Medical Degree
  • Full MRCP or equivalent
Desirable criteria
  • Entry on the GMC Specialist Register for Geriatric and General Internal Medicine via:- • CCT (Proposed CCT date must be within 6 months of interview) • European Community Rights or • CESR

Experience

Essential criteria
  • Evidence of Higher Specialist Training in Elderly Care Medicine (or be within 6 months of confirmed entry from date of interview)
  • Wide experience in Elderly care Medicine equivalent to that required for a UK CCT
  • Ability to offer expert clinical advice on range of problems
  • Ability to take full and independent responsibility for the clinical care of patients

MANAGEMENT AND ADMINISTRATIVE EXPERIENCE

Essential criteria
  • Good organisational and management skills Ability to manage and lead within the Elderly Care Team
  • Ability to organise and prioritise workload effectively
  • Ability to plan strategically and to exercise sound judgements when faced with conflicting pressures
  • Experience of supervising junior medical staff including SpR/StR level
  • Ability to teach clinical and operative skills
Desirable criteria
  • Evidence of management and administration experience
  • Demonstration of leadership skills

CLINICAL EFFECTIVENESS

Essential criteria
  • Ability to apply research outcomes to clinical problems
  • Ability to use the evidence base and clinical audit to support decision-making
  • Evidence of research experience and published work
