Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

We provide high quality, friendly and effective community health services from multiple sites covering Walsall. Our multidisciplinary services include rapid response and homebased care, so that those with long term conditions and the frail elderly can remain in their own homes.

This is an exciting opportunity to join our pharmacy team at a senior level. We are looking for an experienced pharmacist in leading the advanced clinical services in primary care as part of the community division at Walsall Healthcare NHS Trust.

What we are looking for:

You must be a highly motivated Pharmacist who is enthusiastic about providing cost effective, clinically effective and safe patient-focused care.

You will have a track-record of delivering successful operational knowledge of the primary and secondary care interface.

Prif ddyletswyddau'r swydd

Develop and deliver any/all pathways between Walsall Healthcare NHS Trust and partners in primary care.

Participates in the provision of a comprehensive pharmaceutical service (including supporting specialist multidisciplinary teams) to Walsall Healthcare NHS Trust and to those organisations with which the Trust has a contract for supply of pharmaceutical services.

Ensures that the service complies at all times with legal and professional requirements along with appropriate national and regional guidelines.

Responsible for the delivery of a high quality, patient focussed service provided by the pharmacy team in the designated specialist area, and supporting colleagues in its wider delivery across the Trust through clinics.

Provides support to drug budget holders and financial stakeholders within the designated specialist area in order to ensure cost-effective use of medicines and delivery of Cost Improvement Plans. 

Contributes to the strategic development of pharmacy services throughout the Trust.

Provides a high level of pharmaceutical support, advice and information to medical, nursing and other healthcare staff providing clinical care in the designated specialist area.

To manage a caseload of patient within their specialist area.

The pharmacy department at Walsall Healthcare NHS Trust is undergoing some exciting and positive developments with a key focus on the growth of individuals who work here. Therefore, this a great opportunity to be part of this new change.
The Pharmacy Service operates across three acute clinical Divisions and across the Community Services Division provided by the Trust. Applicants will be able to demonstrate significant experience managing a clinical pharmacy service. The post holder will be a key member of the Pharmacy Team working closely with stakeholders within pharmacy, primary care and the community division. 
Applicants must be able to contribute to the delivery of the strategic aims of the Pharmacy Service, aligned to the overall Trust objectives, within set financial constraints as well as deliver agreed cost improvement targets on an annual basis.

1.    To be responsible for the provision of advanced clinical Pharmacy Services within the Community Division, with a specific focus upon Admissions Prevention.
2.    Perform post-discharge reviews according to a work plan. 
3.    To participate in the development of formal medicines optimization pathways between acute, primary care and community Pharmacy services. 
4.    Evaluate service outcomes and financial evaluations to the appropriate service board(s)
5.    To be a fully integrated team member of the Divisional Board and Divisional Quality Team, as well as the designated Care Group team meetings.  
6.    As part of the wider multidisciplinary team, to communicate and manage highly complex clinical information to all grades of healthcare professionals within designated area of responsibility. 
7.    To lead on the implementation and compliance with national guidelines relating to the designated area of responsibility.
8.    To ensure compliance with Trust policies, procedures and guidelines relating to Pharmacy and medicines.
9.    To manage a caseload of complex patients within Walsall.
10.    To conduct independent clinical reviews of complex patients.
11.    To support and contribute to the efficient discharge planning in the designated areas of responsibility and communicating with relevant healthcare practitioners in intermediate or primary care to ensure safe transfer and continuity of care.

  • • Completion of a one year pre-registration programme
  • • Current UK professional registration as a Pharmacist with the General Pharmaceutical Council

  • • Experience in dealing with patients/contractors/stakeholders
