Sepsis and Critical Care Outreach Practitioner

An exciting opportunity has arisen within the Sepsis and Critical Care Outreach team for an innovative, dynamic and enthusiastic individual, who will be part of a team consisting of  critical care outreach practitioners and sepsis practitioners.

The successful candidate will predominantly work  as a sepsis practitioner but will also undertake the role of a critical care outreach practitioner  when  required to meet the service needs.

The successful candidate will be an experienced  critical care nurse with a high level of knowledge regarding sepsis and the deteriorating patient. The Sepsis and Critical Care  Outreach Practitioner  will  work independently, develop good rapport with key leaders and clinicians and have some influencing skills to improve patient outcomes and experiences. They will  be required drive early recognition and management of sepsis and the deteriorating patient at ward level.

To be responsible and professionally accountable for the assessment of care needs;  the  development of personalised plans of care,  implementation and evaluation of care without supervision. 

Ensure the  service promotes  the  early recognition, treatment and management of patients, who maybe deteriorating , or who may have sepsis.

To act in  a manner to  give assurance  that  red  flag  sepsis alerts are responded to  appropriately .

Participate in developing and delivering ward based  training Trust wide  in the early recognition, the management of sepsis and the deteriorating patient.

  • Critical care experience experience.
  • Sepsis awareness and clinical exposure to managing acutely unwell patients

Qualifications

  • RGN post qualification
  • Critical Care Post Registration Course or equivalent 14 months post course

  • Mentorship/SLAiP/supervisor and assessor training or teaching experience
  • Professional updating within last 12 months
