An opportunity has arisen in the NHS@Home team, based at University Hospitals Bristol and Weston Hospitals. With growing evidence that supports the benefits of a ‘home first’ approach for our patients, there has never been a better time to be part of the expanding NHS@Home service.

We are looking for a dynamic, highly motivated, forward-thinking clinical pharmacist to join our small but friendly NHS@Home multidisciplinary team of nurses, pharmacists, physician associates, AHP and microbiology, respiratory and frailty consultants

NHS@Home acts as an extension to our hospitals, enabling patients to go home to continue their hospital treatments in the comfort of their own homes. The Pharmacy role in our team will allow you to continue to utilise and develop your acute clinical and leadership skills.

As the B7 Pharmacist in the NHS@Home team, you will be based at Weston General but will the role will require cross trust working across our Bristol and Weston hospitals to support the management of patients for our Outpatient Parenteral Antimicrobial Therapies (OPAT), Frailty, Respiratory, General Medical and Surgical and Heart Failure pathways.

The band 7 NHS@Home pharmacist will work alongside NHS@Home pharmacy colleagues from UHBW, NBT and Sirona, to deliver and develop the clinical pharmacy aspects of the NHS@Home service within the BNSSG locality.

The specialist role will involve working across boundaries, including primary, community and secondary care, to provide individualised clinical care to patients in the NHS@Home virtual wards.

The NHS @ Home Pharmacy Lead will: 

  • Be an integral member of the team, working with the NHS @ Home team to develop and deliver the service in accordance with Trust policies and procedures, national legislation, and standards.   
  • Work as a specialised practitioner to deliver the clinical pharmacy aspects of the NHS @Home service.  
  • Responsible for ensuring that the pharmaceutical aspects of the service, including prescribing, stability, supply and storage, are delivered in line with relevant national legislation and standards. 
  • Support the Trust antimicrobial stewardship programme, working at all times to ensure practices and guidelines are aligned with those of the antimicrobial pharmacy team, microbiology colleagues and those of the national agenda. 
  • Act as a leader and role model, adopting a flexible approach in a role which encompasses leadership, management, and clinical responsibilities.

Qualifications and Training

  • Master’s degree (or equivalent) in pharmacy
  • Current registration with the General Pharmaceutical Council

  • Membership of the Royal Pharmaceutical Society of Great Britain
  • Postgraduate Diploma (or equivalent) in Clinical Pharmacy
  • Independent Prescribing Qualification / willing to work towards achieving
  • Postgraduate Master’s degree in Clinical Pharmacy or Antimicrobial Management (or equivalent)

Knowledge and Experience

  • Evidence of continuing professional development
  • Evidence of UK hospital pharmacy experience
  • Project working and task management
  • Previous experience of audit
  • Knowledge of basic medicines information and critical appraisal techniques
  • Knowledge of the legal framework for the dispensing, prescribing and administration of medicines

  • Clinical knowledge and experience of antimicrobial chemotherapy and the rational use of antibiotics
  • Experience of one-to-one and group teaching
  • Experience and ability to initiate, lead, manage and supervise a range of projects
  • Experience of managing and motivating staff, appraising and giving feedback

  • Knowledge of basic medicines information and critical appraisal techniques
  • Computer literate, e.g., word processing, use of spreadsheets and presentations
  • Influencing skills
  • Ability to critically analyse prescriptions for clinical appropriateness, clinical risk, cost-effectiveness, adherence to local & national guidance and legality
  • Understanding of medical and nursing practice, record keeping and terminology
  • Ability to work under pressure
