The Macmillan Community Dietetic Service provides nutritional support and home enteral feeding management for people in Buckinghamshire , who are living with cancer or the long-term side-effects of curative treatment.
The post holder will work as part of the cancer multidisciplinary team (MDT), providing specialist nutrition and dietetic services to optimise nutrition and quality of life for cancer patients across Buckinghamshire.
The post holder will provide specialised assessment, treatment and support for patients, many of whom have complex needs and are severely nutritionally compromised.
The post holder will be an autonomous advanced practitioner and act as a source of knowledge and clinical expertise in the area of cancer care, identifying and addressing the educational needs of patients, carers and colleagues.
The post holder will line manage/jointly line manage the Macmillan Community Dietetic Administrator.
Prif ddyletswyddau'r swydd
· To be responsible for the provision of specialised, evidence based dietetic support to cancer patients and their carers.
To provide nutrition and dietary advice for patients diagnosed with cancer and their families, from point of diagnosis and throughout their cancer journey.
To see patients in a variety of community locations across Buckinghamshire, which may include GP/ community clinics, community hubs, and domiciliary visits.
To ensure referred patients are seen promptly in line with cancer pathways and based on clinical need.
To undertake a comprehensive patient centred assessment (interpretation of biochemistry, anthropometrics, clinical condition, diet history and socio-economic circumstance) of patients including those with complex presentations/multi-pathologies and use advanced reasoning skills to set the nutritional goals for each patient.
To identify barriers to change, motivators, personal goals and to use these alongside patients’ wishes to develop treatment plans and provide advice to patients, carers and clinicians
· To contribute to multi-disciplinary discussion and patient assessment / care planning decisions through involvement in MDTs where possible and frequent liaising with teams caring for the patient within BHT and externally.

SKILLS, ABILITIES & KNOWLEDGE

- Skilled in monitoring and developing artificial nutrition regimens for home enteral feeding
- Skilled in assessment for enteral feeding products, equipment and regimens. Evidence of developing and monitoring these in complex patients.
- Accurate anthropometric skills
- Ability to work flexibly and be adaptable to meet the changing requirements and needs of the service.
- Able to deal with distressed, difficult or abusive patients or carers
- Ability to relate and communicate clearly at all levels with patients, colleagues and members of the MDT (verbal / written
- Ability to assist in developing educational resources to meet the needs of patients and carers which are evidence-based
- Awareness of clinical governance and its implications for dietetics including importance of audit projects
- Awareness of regulatory bodies (HCPC) and professional bodies (BDA) and code of practice and professional guidelines Commitment to engage in CPD, appraisal process and clinical supervision.

EXPERIENCE

- Previous clinical experience and detailed knowledge of nutritional support for complex cancer related symptoms
- Substantial post-registration experience in dietetics, including experience at specialist level.
- Experience of supporting other members of the dietetic team with problem solving, learning and caseload management
- Experience of MDT participation and raising the profile of nutritional intervention as required.
- Experience in writing, reviewing and improving guidelines, standard operating procedures and service reports
