An exciting opportunity has arisen for an enthusiastic pharmacist to join our friendly and supportive Medicines Optimisation in Care Homes (MOCH) team within the Community and Rehabilitation Care Group in Buckinghamshire Healthcare Trust. The work of the MOCH team supports to reduce medicines related avoidable hospital readmission from care homes and has improved care across health and social care for the patients of Buckinghamshire. This is an excellent career opportunity for a pharmacist to work collaboratively with community specialist teams, care homes and Primary Care Network (PCN) clinicians, to support the medicines optimisation agenda and to make a real difference.

The post holder will be expected to:

  • work with an integrated, flexible, clinical pharmacy workforce with support functions to deliver complex clinical post-hospital discharge reviews for care home patients to reduce hospital readmission as part of the Trust’s objectives.
  • support care home providers and staff to improve medicines safety, as well as safeguarding concerns as appropriate.
  • undertake guideline development, education, and training, sharing learning and best practices to improve system-wide medicines management processes for care homes and key stakeholders as appropriate.

Edication Qualification and Training

  • Degree in Pharmacy (currently a Master’s degree of 4 years plus 1 year pre-registration training) or equivalent
  • Registered Pharmacist with the General Pharmaceutical Council.
  • Evidence of post qualification education and continuing professional development

  • Postgraduate Certificate, Diploma or MSc in a clinical pharmacy discipline or evidence of extensive clinical training and experience

Experience

  • Substantial post graduate experience in hospital, community or primary care pharmacy
  • Experience of developing and implementing formularies, policies, guidelines and audit
  • Experience of devising and delivering training within health or social care setting
  • Experience of influencing, negotiating and implementing change in practice
  • Experience of managing and motivating a team member and reviewing project performance
  • Experience of identifying and interpreting National policy.
  • Experience of researching best practice (globally, private and public sector), interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform (advising on policy implementation)

Skills, Abilities and Knowledge

  • Clinical expert and knowledge of safeguarding vulnerable adults
  • Sound therapeutic and clinical knowledge in the pharmaceutical care of older people or people with long term conditions
  • Understanding of the implications of National Service Frameworks and NICE guidelines particularly those that impact on the care of older people and those with long term conditions
  • An understanding of the current agenda in the NHS, modernisation and service improvement as it relates to pharmacy and medicines particularly in primary care
  • Understanding of primary care working and organisational structures
