Job overview

The Transfer of Care Hub (ToCH) is a multidisciplinary team which meets daily, in person and virtually, to lead timely and safe discharge decision-making. It aims to deliver proactive and person-centric care through system-wide collaboration which will support people to live well and independently in their community. The ToCH multidisciplinary team reviews patients who have been referred for discharge and make informed and final decisions regarding the most suitable discharge pathway for them.  The ToCH co-ordinates the discharge pathways out of hospital (pathways 1 , 2 and 3)  for residents of  Buckinghamshire, irrespective of where they are an inpatient including out of area hospitals. This also includes  patients who require assistance relating to pathway 0 i.e. Voluntary, Community or Social Enterprises (VCSE).

The post holder will provide senior administrative duties in the ToCH and business support as needed. The post holder will have excellent and experienced skills in communication, administration duties and IT.

Main duties of the job

The post holder will be responsible for the administrative duties of the ToCH such as managing the ToCH referrals and mailbox, screening referrals, updating and monitoring the patient tracker, working closely with system partners to support their referral processes, answering calls, and working with the ToCH Operational lead and MDT. They will liaise with professional staff within designated clinical and social care areas with the overall aim of preventing delays and have support from the MDT as required for appropriate decision making.

The post holder will follow clinical and operational guidelines to support with the implementation of appropriate discharges in a safe and timely manner, to prevent hospital readmissions and ensuring the patient receives continuity of care through clear and timely communication with all involved in their care.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • NVQ3 or equivalent standard of further education
  • Ability to demonstrate an understanding of medical terminology
  • Willing to undergo further training and development if necessary

EXPERIENCE

Essential criteria
  • Experience of working in an office environment
  • Excellent manner and experience of dealing with people on the telephone
  • Experience of patient administration system and other IT system’s
  • Experience of working directly with a variety of staff disciplines
Desirable criteria
  • Experience of implementing process improvements
  • Experience of administrating patient pathways
