Job overview

We are seeking for an Lead for the Home First Service, who is HCPC registered or has equivalent experience and registration in the field of Adult Social care, with extensive experience of  Adults community and acute patient discharge pathways, the service is based at Stoke Mandeville Hospital.

This post is a full time (37.5 hours a week) leadership post that involves management and oversight of the  Home First Service, that supports pathway 1 discharges, from all BHT sites as well as discharges from Wexham Park Hospital for our Buckinghamshire residents. 

This post requires participation is the Transfer of Care Hub processes and meetings, working closely with Care Agencies, patients and their families.  

Successful applicants needs to have extensive knowledge of discharge supporting services, processes and a good track record of high level negotiation skills.  Experience of team leadership and staff management is essential.

 

Main duties of the job

To provide delegated, day to day operational management and professional leadership the Home First service

To work with the Head of Discharge and Flow, Deputy AHP Leads, discharge team, Adult Social Care colleagues, service finding team and commissioners. 

To develop the Home first team in all aspects to be a high performing team, supporting efficient safe discharges as part of Pathway 1 providers.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • Current registration with HCPC or Social Work England
  • Diploma /Degree in Physiotherapy or equivalent
  • Completion of advanced post graduate courses in relevant field
Desirable criteria
  • Professional Membership

EXPERIENCE

Essential criteria
  • Post qualification experience
  • working experience at Band 7 level in Adults discharge and community experience
  • Experience in student clinical supervision
  • Ability to work within an interdisciplinary team and contribute to it’s development
  • Evidence of commitment to continuing professional development, and reflective practice
  • Previous experience of quality issues, audit, research activities and risk management
  • Demonstration of having initiated change of practice or process following identification of clinical or service need
  • Experience of close multi-professional working
  • Establish and Maintain CPD

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria
  • Clear, concise verbal and written communication to ensure liaison with multi disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English)
  • Ability to demonstrate leadership qualities
  • Ability to integrate, participate and contribute as an effective member of a team
  • Ability to plan, manage and organise required tasks within available resources (including time)
  • Awareness of research methodology
  • Knowledge of Current related legislation (H&S, Data P. Act, relevant NSF’s)
  • Experience of implementing EBP
  • Remain updated with advanced professional practice and new research Professional maturity and ability to effectively network and form professional relationships with other healthcare / multi-agency professionals

SPECIAL CIRCUMSTANCES

Essential criteria
  • Able to travel to other sites and off site as necessary
  • Ability to work autonomously
  • Fitness and endurance to work in the physical environment of the clinical area

  • Full Driving Licence
