Job overview
There is something special about caring for people in their own home at the end of their life, this is what as a member of the Florence Nightingale Hospice at Home team you would be doing.
Main duties of the job
· To provide end of life care to patients within their own home.
· Establish effective working relationships with a range of professionals to ensure that care is appropriate and delivered in a timely manner.
· Participate in weekend rotas /7 day working rotas as required

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
- Registered Nurse
- Palliative or Cancer Care degree
- Further training / development to Masters level
EXPERIENCE
Essential criteria
- Extensive post registration experience
- Experience of working in palliative or cancer care
- Experience of managing and working with groups of staff
- Experience of dealing with highly emotionally distressed people
Desirable criteria
- Experience of multi disciplinary working
SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
- Analytical skills in assessing specialist care needs of patients and their families
- Highly developed knowledge of symptom management in specialist palliative care
- Ability to share specialist knowledge in an informal and formal setting
- Highly developed communication skills
- Provide support to patients and their families in extreme emotional distress and to the staff looking after them
- Able to communicate with the multi disciplinary and multi agency team and offer specialist advice.
- Able to motivate self and others
Desirable criteria
- I.T. skills
SPECIAL CIRCUMSTANCES
Essential criteria
- Ability to travel trust wide
