Job overview

The post will involve working with healthcare assistants and clinicians to provide nursing interventions to patients in their home environment, maximising their independence and providing care to avoid hospital admission as well as facilitating discharges from the acute.
You must be innovative in your approach and willing to work with other members of the integrated team and ward staff to ensure a high quality service delivery. You must be willing to develop your own knowledge and skills, you will be supported in your personal development with training to extend your community nursing skills.
Experienced and newly qualified staff nurses will be considered for the post. 7 days a week shift pattern
Main duties of the job
- To contribute to processes for service and quality improvement
- Assess the physical and psycho–social needs of patients and deliver
therapeutic interventions based on best available evidence in order to
improve health outcomes. - To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
- To plan interventions with the Case Manager and other team members using the common assessment process
- To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/service
- To contribute to the development of protocols and procedures developed for clinical pathways

Education
Essential criteria
- Registered Nurse or student nurse awaiting confirmation of NMC registration.
Desirable criteria
- Post registration education and extended skills eg Intravenous drug administration , compression dressings
Experience
Essential criteria
- Demonstrate experience post or pre-registration placements in relevant clinical areas: Care of Elderly Community Orthopaedics Neurology Respiratory Palliative Care Long term conditions
Desirable criteria
- Knowledge of working with a single assessment , planning and the delivery of care
- Experience of electronic clinical systems and technology eg RIO
SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
- To be able to communicate effectively with patients, relatives, carers and other professionals.
- Able to work under pressure
SPECIAL CIRCUMSTANCES
Essential criteria
- Car driver / owner able to travel to other locations within the trust
