Job overview

The main purpose of this role is to support the Clinical Nurse Specialist (CNS) and the urology team in coordinating patients’ care with a diagnosis or suspected diagnosis of urological cancer during the diagnostics, treatment and surveillance.

Main duties of the job

  • To work closely with the nursing and non-nursing team under direct supervision.
  • To provide an administrative support to the CNS and urology team.
  • To coordinate patients’ care on specific cancer pathways.
  • To support patients under the CNS and urology team.
  • To act as a point of contact for patients with pre and post cancer diagnosis.
  • To manage and to coordinate patients on a Personalised Stratified Follow-up (PSFU) pathway.
  • To capture patients’ activity on relevant hospital electronic systems.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • GCSE English Language and Mathematics or equivalent
  • Assistant Practitioner Foundation Degree in Health and Social Care or NVQ 4 equivalent
  • Undertaken the HCA course (The Cancer Journey Caring for the Cancer Patient and their Family) or willingness to undertake
  • Keyboard and computer literacy skills

EXPERIENCE

Essential criteria
  • Relevant health or social care experience at AfC Band 3 or equivalent
  • Experience of multi-professional working
  • Experience in the use of data management
  • Evidence of good communications skills
  • Understanding of person-centred care
  • Understanding of the health and social care environment
  • Awareness of the NHS long term cancer plan and personalised care, including eHNA
Desirable criteria
  • Experience of coach/teaching patients and carers
  • Experience in coordinating a patient workload
  • Awareness of cancer policies
  • Knowledge of relevant cancer treatments, interventions and terminology
