Job overview
An exciting opportunity has arisen for a rehab support worker in our Early Supported discharge team.
This is a very rewarding and varied role working alongside stroke patients who have recently been discharged from hospital. The team aims to provide high quality stroke rehabilitation and you will have the opportunity to work with patients in their home environment. You will be involved in carrying out rehab programmes under the supervision of the team therapy staff and also supporting the team with some admin duties.
You will work alongside and support our multidisciplinary team including Occupational Therapists, Physiotherapists, and Speech and Language Therapist. Previous experience in the healthcare setting would be valuable. We are looking for someone with a positive outlook who is able to motivate and encourage patients to achieve their goals.
This is a friendly, supportive and dynamic team with integrated individual supervision, appraisal and opportunity for CPD and in service training.
Main duties of the job
- To support qualified Therapy staff by implementing and undertaking designated patient assessment and treatment programmes.
- To document, evaluate and feedback to qualified Therapists following all interventions.
- To support the ESD team in non-clinical tasks.

- Clear, concise verbal and written communication to ensure liaison with multi disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English)
- Ability to learn / ability to self initiate learning (evidence within the last 2 years)
- Able to know own limitations and seek advice.
- Flexible and adaptable in approach to work.
- Able to undertake manual handling tasks.
- Physical ability and endurance to work in the physical environment of the clinical area required by this post

- Driving licence and ability to drive / travel on occasions to other locations in Bucks and other FNP sites
