The Community Respiratory Nurse Led Service are highly skilled teams who provide autonomous care to patients with chronic respiratory disease.  We are very supportive and have an excellent staff retention rate with high staff morale.

We have a rare vacancy for a Band 7 Respiratory Team Lead  to join us. This post will be based in Cambridge however the successful candidate will be required to work flexibly across the countywide respiratory service.   So, if you want to be part of a friendly, dynamic, forward thinking team who enjoy coming to work each day please contact us.

This is a full time post, however, part time hours would be considered for the right candidate.

The post entails providing expert supportive care for respiratory patients, by helping to prevent avoidable hospital attendances, providing facilitated support following hospital admission and the promotion of focused self management, which will include reviewing those with home oxygen therapy.

 

The successful candidate must be capable of working autonomously whilst supporting the local Cambridge respiratory team, and work with the support of CPFT respiratory colleagues, in providing a seamless service of care. The post holder will work in collaboration with other services across the CPFT and other stakeholders in order to make a positive impact on the health and well-being of patients with chronic respiratory disease. They will also act as a community respiratory care resource for patients and other health professionals.

  • To work as a member of the respiratory team to undertake advanced assessment, treatment, monitoring and evaluation of complex respiratory caseload; working seamlessly across primary and secondary care boundaries to facilitate prevention of unnecessary hospital admissions.
  • Management of advanced respiratory disease, through the promotion of self management and coping skills; exacerbation management; oxygen assessment and monitoring; quality end of life care
  • The development of links with patients and other members of the multidisciplinary team to ensure patient pathways are robust and fit for purpose
  • Management of the staff in the team and also working collaboratively with other team leads to ensure countywide service provision
  • Implementation of National and local policy directives in conjunction with clinical lead
  • Participation in multidisciplinary team (MDT) working
  • Provision of specialist advise and education to colleagues and other members of the MDT
  • To manage the day to day running of the local respiratory team, ensuring that resources are used efficiently and effectively; providing support and encouraging development of staff

 

 

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
