We have a great opportunity for you to join our Children's Community Specialist Nursing (CCN) Team; You will be leading holistic assessment, care planning and review of clinical care for children and young people with a range of health needs to optimise their health outcomes.
All our CCN's take the lead on an area of community clinical practice e.g. IV's, enteral, palliative care, respiratory, oncology, It is important for you to develop and continually influence good quality practice in the community and with partner organisations.
We recognise it may feel daunting to move into a leadership role in the community setting, where you'll be expected to lead and work independently but you will be working with a supportive team who will guide and support you.

- Clinical competence to deliver clinical nursing care independently and respond to unexpected demands and scenarios in a variety of community settings.
The ability to travel daily during the working day sometimes at short notice is therefore essential.

Provide skilled paediatric nursing care to children and young people within their own home, or other applicable venues such as outpatient clinics, to ensure families are supported in the care of their children at home.
Ensure continuity of care is established and appropriate support available for children, young people, and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge where appropriate from both local and tertiary hospitals.
Act as a named nurse for a caseload of generic children and young people responsible planning and co-ordinating care, delivering and evaluating the care given. Must use own initiative and work autonomously to provide skilled care. Supervision and support available from Team Leader and senior CCN.
Complete accurate client records in line with CPFT policy. Be aware of the requirements of both paper and electronic record keeping.
Maintain excellent communication with the child and family on the caseload and as their advocate ensure that they have ease of contact with the team appropriate information and support. Records should reflect the child’s care, present condition and progress and be available to colleagues.
Promote the health, comfort, dignity, privacy and quality of life for children and young people.
