Cancer Tracker - Peterborough

We are currently seeking a highly organised and proactive individual to join our team as a Cancer Tracker. The primary purpose of this role is to manage aspects of the Head and Neck cancer PTL (Patient Tracking List) by actively managing the cancer pathway, monitoring, reporting, and tracking cancer patients, and ensuring all relevant information is added to the Cancer database.

Prif ddyletswyddau'r swydd

As a Cancer Tracker, you will work closely with both clinical and administrative staff, as well as other health and social care providers, to manage the inter-trust cancer pathway. Your main duties include monitoring, reporting, and tracking cancer patients to ensure timely and efficient processing of their journeys according to the East of England Best Practice Pathways and National Cancer Waiting Times Targets. You will resolve pathway delays promptly and escalate issues as needed.

Additionally, you will support the MDT Coordination Team by providing administrative assistance to the cancer S/LMDTs, ensuring all necessary paperwork and clinical information is available. You may also attend MDT meetings to record attendance and outcomes. Ensuring the efficient transfer of patients and information between trusts is critical.

Data collection and analysis are integral to the role. You will collect data prospectively using the Somerset Cancer Register data collection system, maintain accuracy, and analyse target breaches, reporting findings to the Cancer Performance Manager and Lead MDT clinician.
