Memory Assessment Service Team Leader

We invite you to apply for North Memory Assessment Services (MAS) Team Lead.

We are offering an exciting opportunity to be the lead for Huntingdon, Peterborough and Fenland MAS that currently delivers assessment for dementia diagnosis using home visits, clinic attendance and by exception remote technology. The successful candidate will have strong organisational skills to enable the services to flex with the changing requirements of offering dementia diagnosis and post diagnostic support. The Service model is closely aligned with MSNAP principles and has a high volume of referrals.

The team consists of Psychiatrists, Neurologist, Nurse practitioner and a Nurse consultant. There is close working with Psychology / Occupational Therapy and some co-location with the Alzheimer’s society and social care. We are looking for someone with a keen interest in dementia and older people’s care with the necessary organisational skills to coordinate a geographically dispersed service.

-The post holder will work with the locality mental health teams in conjunction with stakeholders including the General Practitioners, core team members, specialist services and partner agencies from within and beyond the organisation.

-To ensure that memory assessment pathway is delivered as a diagnostic service for Dementia and to ensure smooth communication and transitions within local services.

-Undertake and coordinate assessments to determine patient needs.

-Collaborate with key partners and stakeholders including local authorities and independent/private sector providers to introduce dementia medication where appropriate and meet the needs of patients and their families/carers whilst working at all times to reduce risk.

-To ensure that adults and older people experience a timely, effective and person centred approach.

-To manage and provide clinical supervision and leadership to a defined group of staff working within the specified localities, within the Memory Assessment Service (MAS).

-The post holder will support the development of skills in relation to Multi disciplinary working, discharge planning/transfer and co-ordination of care in order to achieve a standardised approach and competency.

-To contribute to the management and coordination of the community mental health response, including triage and referrer liaison.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.   •  Work within the model of the MAS providing clinical leadership on the matters relating to mental health interventions.
  •  Clinically supervise mental health practitioners within the MAS and ensure that there is excellent communication and planning for mental health interventions and discharge.
  •  Ensure systems are in place to receive and manage referrals to the team and delegation to staff members with the level of skill and competence required to meet patient need.
  • Provide leadership on the assessment and care planning/ interventions for patients whose presentations may frequently be complex and multi-factorial in nature. Collaborate with the integrated team, Advanced Practitioners/ medical colleagues, and other stakeholders involved in care.
  •  Undertake triage and reviews for appropriate patients; being responsible for ensuring care plans are in place; have been delivered; and complete relevant documentation.
