Trosolwg o'r swydd
An opportunity for a Band 6 Dementia Care Advisor has become available. This role will cover both the REACT and CMHT teams within Mental Health Services for Older People (MHSOP). Both teams are multi-professional with representation from psychiatry, nursing, psychology and therapy staff.
REACT is an enhanced home treatment team that supports adults over 65 and their carers/families on an intensive basis to treat them in the community. REACT aim to provide evidenced based interventions to avoid admission to hospital or to facilitate early discharge from hospital.
CMHT is an integrated (NHS and Local Authority) community mental health team for adults over 65 with severe and enduring mental health issues that present with a high level of complexity, need and risk. CMHT provide care co-ordination and specialist interventions for people who fall into this category.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Prif ddyletswyddau'r swydd
The expectations of the role of Dementia Care Advisor include:
Specialist assessment and evidence based advice and interventions.
Collaborative working with the Multi Disciplinary Team.
Knowledge and expertise on resources available to promote independence of the individual with dementia and their carers.
Advocating for person centred care for people with dementia and their carers.
Psycho educational support and role modelling approaches to care givers.
To be able to use their expert level knowledge of dementia to provide education and guidance to other team members on approaches to best support the individual and carers.
Knowledge and understanding of legal frameworks and how these apply to individuals i.e. Mental Capacity Act 2005/Mental Health Act 2007.
Both the REACT/CMHT teams are based at Llanfair Unit, Llandough Hospital. The successful applicant will be required to travel across the Cardiff and Vale area on a regular basis to see individuals and carers in their home environment.

- First-level mental health nursing or registered health professional
- Relevant post-graduate degree to at least diploma level
- Evidence of professional and management development through Continuous Professional Development

- Relevant post-graduate qualifications – psychological interventions
- Masters degree in relevant area
Experience
Meini prawf hanfodol
- Sufficient experience working at Band 5, (or equivalent level in other disciplines), including experience in a clinical area related to care of the older person in a mental health context.
- Experience of inter-professional and multi-agency team working.

- Experience in Older People’s CMHT service or Crisis and Home Treatment Team.
Skills

- Well-developed skills in terms of managing clinical scenarios that are complex, challenging and have high levels of emotional content
