We have a fantastic opportunity for an experienced Advanced Clinical Pharmacist to join our friendly, established and respected Rheumatology Team across Hywel Dda University Health Board.
The successful candidate will work closely with the Rheumatology team, pharmacy team and homecare department.
This role requires the management of homecare prescriptions for rheumatology receiving biologic treatment. You’ll be required to work closely with the homecare department to ensure that rheumatology patients receive their biologic treatment in a timely manner with appropriate monitoring. This will also require management of any biologic drug shortages or biosimilar switches. You will be required to answer homecare queries relating to the management of our rheumatology patients and support the Rheumatology team (Consultants and specialist nurses) with medicines queries and prescribing.
The post is advertised for 37.5 (Full time hours), 30 hours will be supporting the Rheumatology team and 7.5 hours will be working within the pharmacy department. The role will require the successful to participate in the out of hours on-call pharmacy rota.
Part time hours / flexible working also optional.
Main duties of the job
- Developing close working relationships with other members of the multi-disciplinary team and contributing to clinical care plans
- Responding to medication-related queries, providing advice on prescribing and promoting evidence-based practice
- Discussing medication with patients and carers and developing a variety of approaches to support informed care
- Educating colleagues about medicines management and medicines optimisation
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications and Knowledge
Essential criteria
- Masters degree in pharmacy
- Registered with the General Pharmaceutical Council.
- Postgraduate MSc or Diploma in area of practice or equivalent experience or qualifications
Desirable criteria
- Accredited Preregistration tutor or pharmacy diploma tutor
- Independent Prescriber
- Aseptic Preparation and Dispensing of Medicines (Leeds University) or equivalent if role includes aseptics
- Registered as QC if role includes aseptics
- Member of the Royal Pharmaceutical Society
Experience
Essential criteria
- Specialist knowledge of medicines management systems and process in an integrated NHS.
- Knowledge and experience of healthcare
- Specialist knowledge of own field of practice gained through diploma level training and experience
Desirable criteria
- Demonstrate specialist knowledge in more than one field
- Practice Research Experience
- Training and education of pharmacy staff and other HCPs
- Experience of managing change
- Experience of service improvement
Language Skills

- Flexible approach to needs of the service to undertake on call work, weekend and evening opening.
