We have a vacancy in our Complex Needs Service on the Wirral for a  Mental Health Practitioner.

This is an exciting role and very rewarding work.

The service's remit is improving outcomes for people who have high levels of service utilisation who present with severe personality disorder. 
The objectives of the role are to increase the clients’ stability, reduce levels of service utilisation (particularly urgent and unplanned care), broadly improve mental health outcomes, increase the likelihood of the person’s engagement in formal, planned mental health therapy treatments so that discharge may take place in a planned, recovery focused way.

Wirral Complex Needs team is a fantastic team to work for. We are a small team working with a caseload of clients with personality disorder delivering psychological therapy. We look forward to your application

**We offer a relocation package of up to £8,000**

 


The relocation package is for anyone who is joining CWP in a clinical role at Band 5 or above, who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.


To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

  1. Manage a caseload and work within relevant codes of practice at all  times
  2. Undertake individual service user focused specialist assessments on all referrals received by the team. 
  3.  Use skills for assessing, interpreting and appropriate diagnosis of mental health service users conditions and carrying out appropriate action.
  4. Signpost to the appropriate service to meet individual service user and carer needs.
  5. To deliver evidence based interventions which are measurable and which promote the service users journey along the recovery pathway.
  6. Provide training in specialist area.

  • Knowledge of delivering individual service user focused specialist assessments
  • Knowledge of evidence based interventions which are measureable and which promote the service users journey along the recovery pathway
  • An in-depth knowledge of delivering psychosocial interventions (PSI)
  • Knowledge and understanding of physical health needs
  • Professional knowledge acquired through a relevant degree plus relevant experience for working in the community
  • Highly developed accurate physical skills and be able to manipulate fine tools and materials
  • Knowledge of developing specialist programmes of care, care packages
  • Knowledge of providing specialised advice in relation to care
  • Knowledge of recording personally generated information
  • Knowledge of working closely with commissioners and care providers
