As the NHS progresses with the implementation of the Patient Safety Incident Response Framework (PSIRF) Cheshire and Wirral Partnership NHS Foundation Trust (CWP), is demonstrating its commitment to providing high quality care and are currently looking to expand our team of dedicated Investigating Managers.

 

We are looking for 3 individuals to join a centralised team of investigating managers working within CWP. You will be working with Specialist Mental Health Services across Cheshire and Wirral including inpatient, community and first response services. The post holder will work alongside a wide variety of colleagues including nursing and medical staff, operational managers, governance teams, Directors and patients, carers, and families.

 

Investigating Manages will lead on a variety of investigations including system-based patient safety incident investigations, complex complaints, and human resource investigations and act in the role of an engagement lead for persons involved in a patient safety event. We are also committed to supporting patients, carers, families, and staff to have meaningful involvement throughout the process.

Main duties of the job

Application will be welcomed from candidates who are to be able demonstrate

 

·       Being able to be work autonomously and to build relationships of trust when undertaking investigations.

 

·       Demonstrate organisational and time management skills to be able to prioritise workload and meet agreed time frames.

 

·       Compassionate engagement and involvement of those affected by patient safety incidents with the ability to communicate complex matters in difficult situations.

 

·       Adopt a just culture and being open and honest and giving apologies when necessary.

 

·       The competency to apply system based analytical skills to undertake sensitive and complex investigations.

 

·       Ability to understand and interpret the interactions between person, the environment, tools and tasks, the organisation, and external factors

 

·       Effective interviewing skills that maximise quality responses, to gain understanding of why something did or did not happen and how system factors interacted or impacted on what, how and why.

 

·       Experience of writing high quality, precise, inclusive and person centred, reports that focus on systems and not on individual blame.

 

·       Identify learning, recommendations and, to collaborate with teams to facilitate SMART actions to support improvements and improve safety.

Detailed job description and main responsibilities

To undertake investigations and other types of learning response related to serious patient safety incidents, complex complaints, and complex Human Resources (HR) investigations on behalf of the Trust. The post holder will:

 Ensure each Patient Safety Incident Investigation (PSII) or other learning response, complaint and HR investigation is conducted in full and to a high standard using appropriate methodologies, whilst ensuring that the principles of being open and duty of candour are applied.

Compile complex reports outlining the findings of investigations and develop recommendations in conjunction with the relevant managers, clinical and speciality experts of the service involved and Trust wide if appropriate.

Support the Trust in the process of analysing and embedding learning across the Trust.

Qualifications

Essential criteria
  • Educated to Masters level (or equivalent experience) in a relevant field
  • Investigations/learning response (e.g. systems based investigations) training from an accredited provider (or commitment to complete upon appointment).Evidence of post qualifying and continuing professional development.
Desirable criteria
  • Member of relevant professional body
  • Human Factors Training
  • Clinical Qualification
  • Quality improvement training
  • Completed level 1 (essentials of patient safety) and level 2 (access to practice) of the patient safety syllabus (or commitment to complete upon appointment).

Knowledge and Expertise

Essential criteria
  • Demonstrable and extensive investigation and learning response expertise.
  • Considerable knowledge and demonstrable experience in using system thinking principles (e.g. SEIPS)
  • Good understanding of Microsoft packages with competent word-processing skills.
  • Robust understanding of the background to and aims of current healthcare policy, including National Patient Safety Strategy and appreciate the implications of this on engagement.
  • Comprehensive knowledge of the National Patient Safety Incident Response Framework
  • Detailed knowledge of Coroner’s Court process and procedures.
  • Knowledge of the Mental Health Act.
