Salaried GP with a number of combinations of sessions offered.
The division has the pleasure in offering an innovative and fresh opportunity for a salaried GP to join the Hospital at Home service supporting people with medical interventions at their own home.
As pressure on emergency departments across the UK grows, new care models will be pivotal in delivering and relieving some of these pressures by diverting work away from the hospital and into the community.
This is an exciting opportunity to work collaboratively with other multi-disciplinary teams within primary and secondary care and to expand your portfolio and leadership skills in evolving primary care roles.
Hospital at Home (Clinical sessions-negotiable)
Our Hospital at Home service is designed to provide monitoring and treatment at home, which has traditionally been provided in hospital, such as intravenous (IV) therapy. The service encompasses Virtual Wards, Urgent Community Response (UCR) and IV OPAT service. The post holder will be required to undertake medical management including clinical assessment ensuring safe management of patients within the Community setting. Working closely with primary and secondary care colleagues to ensure safe and effective clinical care is delivered to support admission avoidance and facilitate early discharge where clinically appropriate.

1. Manage the medical requirements and provide medical oversight to patients being cared for by the intermediate care service (Hospital At Home).
2. Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral to other agencies where appropriate.
4. Develop, monitor and adjust clinical management plans for health in consultation with patients and other team members and in line with current pathways and protocols
5. Liaise with secondary care consultants in the management of patients within the intermediate care pathways.
6. Undertake and order appropriate clinical diagnostic testing including arterial blood gas analysis, Xrays and blood tests.
7. Be competent in arterial blood gas interpretation and ECG interpretation.
9. Assist in developing appropriate pathways for people in intermediate care settings and for people being cared for by the team to avoid unnecessary hospital admission.
10. Assist in developing appropriate pathways for people in secondary care setting to facilitate safe early discharge into intermediate care service.
11. In accordance with the service rota as agreed, the post-holder will undertake clinical sessions including, telephone advice and visiting patients at home. Required shifts are weekdays and there is an expectation to join the GP on-call rota at the weekend.

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:
- The Countess of Chester Hospital: providing 438 general and acute beds
- Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate and outpatient facility
- Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
The Trust employs over 6258 staff (headcount) which includes temporary bank staff and provides acute emergency and elective services, primary care direct access services and obstetric services to a population of approximately 407,000. This includes 357,000 residents in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire which has a population of approximately 50,000.
The Trust is a busy district general hospital and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £23 (standard) or £43 (enhanced), this cost will be deducted from your salary over the first three months of employment.

- medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003
- Not subject to suspension under section 41A of the Medical Act 1983
- Qualified General Practitioner (completed certificate of Completion of Training – CCT)
- Currently on a CCG performers list and not suspended from that list or from the medical register
- Have had an annual NHS appraisal

- MRCGP
- DCH
- F2 supervisor training/teaching diploma (this is essential for HDS)
- Evidence of CPD activities
