Ellesmere Port Hospital has 59 beds across three rehabilitation units. It is our aim to deliver person centred care, in the right place, at the right time.

We would welcome applications from both newly qualified nurses and those with experience with an interest in care of older people. We are seeking Individuals who are comfortable to challenge change and are committed to improving patient experience.

The Intermediate Care Units have a different focus on care and provide a person centred approach with the aim of optimising physical health and wellbeing over a 24 hour period.  Our units provide short term general and stroke rehabilitation for patients who are medically optimised but not at a suitable level for discharge home. Patients will transfer from emergency medicine, current inpatient beds, repatriation for a period of rehabilitation as well as step up from the community.

The nursing role will be to support patients to manage their long term conditions and act as an autonomous practitioner. This role would develop your autonomy, discharge planning skills and relationships with community and 3rd sector services alongside being in a proactive and supportive team.
