CCICP Community Pharmacist

CCICP have an exciting opportunity for a Community Lead Pharmacist to undertake a 12 month secondment within the South Cheshire and Vale Royal locality. The successful candidate will have the opportunity to  lead on patient safety and clinical governance initiatives in relation to medicines management across Community services. The post holder will also take responsibility for being the community lead for non-medical prescribing.

CCICP was formed in 2016, since this time CCICP's workforce has risen to over 1000 staff working across 49 different services which span across 33 different sites. Our 2023 National Staff Survey demonstrated that 97% of our staff felt trusted to undertake their job and 89% of colleagues feel that care of patients/service users is the organisations top priority. Mid Cheshire Hospitals NHS Foundation Trust provides a full range of hospital and community services for people across East Cheshire (population 399K) and West Cheshire and Chester (population 357K). The Trust provides high quality planned and emergency care, cardiac, critical care, child health, maternity services and intermediate care at Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

1.              To manage the planning and development of systems of patient focused pharmaceutical care to CCICP and to help plan further development of clinical pharmacy to this area including service developments

2.              To manage the development of clinical pharmacy and provide a clinical pharmacy service to CCICP. This will include:

·       Development and review opportunities for clinical and non-clinical staff within the division.

·       Interpretation and implementation of national guidelines at a local level.

·       Development of evidence-based guidelines and policies

3.              Participate in multidisciplinary meetings and integrated governance meetings relevant to community.

4.              Ensure seamless pharmaceutical care across the primary / secondary care interface.

5.              To ensure that medicines information and guidance forms part of the patient safety and governance systems within the community

6.            Initiates, conducts and supervises relevant practice research, drug utilisation review and audit programmes, and leads pharmacy participation in clinical trials relating to the community.

7.     Develops and maintains systems of risk management including reporting of incidents and periodic reporting of trends.

8.       Provides information to prescribers and other healthcare staff to enable progress and future planning.

9.       Reacts to public opinion on treatment of patients with medicines, managing the introduction of improvements.

10.           Maintains continuing professional development.

11.  Accountable for professional actions, including those where information is lacking, medical opinion is divided, and/or where advice may be challenged by other healthcare professionals.

12.          Development of strategy in response to NHS guidance in relation to the division.

13.        To assist the division to manage their drug budget. Responsibilities will include:

·     Providing regular detailed evaluated reports on drug expenditure to divisional boards.

· Forecasting new drug developments and identifying cost pressures.

·    Evaluating cases to support applications to the Joint Medicines Management Group.

14.          Ensures compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care. 

15.   Overall responsibility for good practice in the selection and prescribing of medicines throughout the division including analysis of adverse drug reactions.

16           Managerial responsibility for staff providing pharmaceutical services to the division, including recruitment, appraisal and job plans.

17.       Responsible for the training of medical, nursing staff and non-clinical staff in the prescribing, administration and use of medicines within the division.

18.    Maintains a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.

19.       Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.

20.      Using outcome of practice research to plan education systems within community services.

21   Designs and provides education and training to healthcare staff including medical, nursing and pharmaceutical staff
