We have an exciting opportunity for an experienced nurse/physiotherapist with an enthusiastic interest in Adult Respiratory Medicine. We are a seven-day service consisting of a specialist team of Nurses and Physiotherapists who work primarily to meet the needs of patients with long-term respiratory conditions. 

The role will include conducting face to face assessment and reviews of both long-term respiratory conditions and acutely unwell people within their own homes, hospital, and a clinical environment. 

This role will help lead the role out of the Respiratory Virtual Ward and be a key contributor to this new and exciting development in supporting patients in their own home. 

We also provide the home oxygen assessment and review service for all adult patient regardless of diagnosis.

You will act as an autonomous practitioner within the specialist team providing both community care and in-reach into the acute trust. This includes a mixture of face to face and telephone assessment of people with Respiratory or long-term conditions. You will be flexible to meet the needs of a reactive service and should be able to prioritise your workload based on patient/service need.

You need to be able to perform under pressure within an extremely busy service and have the necessary skills to diagnose, treat and respond to the changing needs of the service as it continues to evolve.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

We employ almost 5,000 members of staff, provide around 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.

The Trust is also part of Central Cheshire Integrated Care Partnership (CCICP), a unique local health partnership that provides a range of community services for people across South Cheshire and Vale Royal.

During the Coronavirus pandemic, the Trust was recognised nationally for its ‘Be Safe Be EquiPPEd’ campaign, which aimed to make the Trust as safe as possible for staff and patients, through supporting the correct use of PPE.

The results in the 2020 national NHS Staff Survey showed improvements in all elements of the safety culture theme, with 92% of respondents feeling their role made a difference to our patients.  We were also recognised nationally for our workforce health and wellbeing initiatives.

JOB PURPOSE

  • To undertake in-depth specialist clinical assessment, diagnosis, and treatment for patients with respiratory conditions.
  • To support in leading the specialist nursing/practitioner function in the delivery of specialist care across the integrated respiratory team including assessment of complex care needs and an ability to lead in developing and implementing programmes of care.
  • To take a lead role in facilitating, enabling, and supporting the development of the Integrated Respiratory service based on best practice ensuring delivery of high quality, effective and efficient care within this specialty.
  • To participate in the education and development of staff members.
  • To lead in the development and delivery of high-quality patient centered care and working with the associated clinical team to ensure this is incorporated within the Business Group and Trust objectives.
  • To act as an expert resource for all those involved in respiratory care across the Primary and Secondary settings.

 

SERVICE DELIVERY

  • Supports the planning and development of clinical protocols for specialist area.
  • Works autonomously to manage his/her own caseload of patients whilst working as part of a multidisciplinary team without direct medical supervision.
  • Enables patient choice and involvement and initiates appropriate action.
  • Coordinates and develops patient care pathway and liaises across organisational boundaries.
  • Undertakes specialist assessment and plans specialised programmes of care and provides highly specialised advice to patients, relatives, and carer’s.
  • Ensures each patient is placed in the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised.
  • Provides clinical advice and support to healthcare professionals within and across organisational boundaries.
  • Provides clinical advice in accordance with current evidence-based practice.
  • Undertakes nurse/practitioner -led clinics.
  • Participates in case conferences in relation to future management or discharge arrangements.
  • Ensures patients and their families are given appropriate oral and written information regarding planned care.
  • Maintains accurate patient records and ensures all relevant information is documented in the patient’s medical and nursing records.
  • Work as an independent non-medical prescriber (adhering to Trust policy) with the ability to initiate and adjust medication within your field of expertise.
  • Participates in MDT with consultants and specialist registrars to discuss complex case management

 

 

GOVERNANCE

 

  • Be a member of relevant professional association and attend/present at regional/national meetings and conferences as required.
  • Identify personal learning needs and take steps to pursue personal development plan
  • Maintain practice within the context of the professional guidelines for the relevant professional body and policies laid down by the Trust.
  • Maintain a comprehensive personal profile and demonstrate appropriate professional development to Post Registration Education and Practice standards.
  • Evaluate professional performance and identify implications for practice.
  • Ensure the promotion of safety, well-being and interests of patients, staff and visitors to the department.
  • Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.

 

MANAGERIAL/LEADERSHIP

 

  • Manages own caseload, and coordinates patient journey across business groups and organisational boundaries.
  • Works in close collaboration with other health care professionals across organisational boundaries to ensure service meets patient’s needs.
  • Supports the management of the Clinical Team in the day-to-day delivery/management of services including co-ordination, delegation, and supervision of the team to facilitate effective service delivery.
  • Assists with the recruitment and initialising of new staff.
  • Acts as a clinical supervisor to staff within the department.
  • Acts as a mentor to pre and post registration nurses, assisting in meeting their learning needs.
  • Lead/support in the development of local policies, protocols and procedures in accordance with local and national standards in relation to specialist area of expertise
  • Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.

 

EDUCATION/LEARNING

 

  • Lead/support the development, planning and delivery of a range of specialist educational and training programmes for healthcare professionals, patients and carers.
  • Lead in the teaching and development of healthcare professionals, patients and carers.
  • Provides clinical supervision and mentorship to junior or new clinical nurse specialists / practitioners/nurses / medical staff as required.
  • Identifies assists and meets the educational needs of the patient and their family.
  • Maintains own professional development, skills and competencies through participation in training and development activities.
  • Maintains and updates evidence based knowledge and skills.
  • Fulfils all mandatory training requirements for the trust and assists others within the department to do so.
  • Undertakes health promotion across organisational boundaries.
  • Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
