Job overview

Are you an experienced health professional who enjoys working in a multi-professional team . If so an exciting opportunity has arisen to work alongside the Ellesmere Port West Community Care team as an Enhanced Clinical Practitioner. 

As an enhanced clinical practitioner you will work alongside the community team to manage a caseload of patients with a range of medical conditions, and act as an escalation point for patients who are being managed at home to GP, hospital or Hospital at Home. You will have close working relationships with specialist teams such as respiratory, frailty, palliative care, heart failure as well as with community teams.  

**We offer a relocation package of up to £8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Main duties of the job

As a key member of the clinical team, you will be expected to provide clinical examinations, clinical case management, case reviews and continuing health care reviews. 

Developing good relationships with the wider MDT to promote patient care and referring to other health professionals and external agencies as required to best support the patient. 

Successful applicant will be professionally and legally accountable for all work undertaken within the role and have professional autonomy and accountability within CWP policies and NMC or Health and care professions council code of conduct. 

To hold or be working towards V300. 

To provide senior clinical leadership within the community care team and support the team manager where appropriate. 

To support staff with professional development and provide mentorship for students and new starters within the team.

Qualifications

Essential criteria
  • Registered Nurse Level 1 or qualified AHP
  • V300 Prescribing or working towards this study
  • Mentorship & Assessing Qualification
  • To have completed the level 7 clinical examination skills module or be currently undertaking this study
  • Post holder requires minimum 5 years post registration
  • To be working towards a level 7 qualification in an appropriate area of clinical practice

Knowledge and Expertise

Essential criteria
  • Evidence of continuing professional development

Experience

Essential criteria
  • Current experience in assessing and treating in a community setting.
  • Current experience working as an autonomous practitioner conducting fact to face consultations
Desirable criteria
  • Experience in telephone consultations
