There is an exciting opportunity within the Mental Health Access Team  for a  full time  permanent Band 6 Mental Health Nurse to join our dedicated  Team covering the West Cheshire region. 

We are looking for an enthusiastic and motivated individual with excellent communication skills, who has the ability and confidence to work autonomously as part of a multi disciplinary team.

The Team is based in Ellesmere Port and may require some travel to and from clinics in the Western Cheshire location, however there is also opportunity for some remote/home working.

The service covers the hours of 9am-5pm Monday to Friday, however there is opportunity for flexible working hours, dependent upon service  needs. 

Main duties of the job

The post holder will carry responsibility for the triage, assessment and review  of patients  who require interventions within Primary Care or onward referral to appropriate services.

As a single point of access service, the workload is rich and diverse, including assessment of mild/ moderate mental health difficulties,  referring onto Specialist Services, including working closely with Talking Therapies  colleagues as well as assessment of complex and severe mental health problems and referring on to appropriate services, depending on needs and complexities.

You will work as part of an experienced MDT, including Mental Health Nurses, Consultant Psychiatrist, Social Prescriber and Liaising with other teams.

Person specification

Qualifications

Essential criteria
  • NMC Registered Nurse RMN/ RNMH educated to diploma / degree level.
  • Specialist knowledge in terms of primary care mental health
  • Evidence of continued professional development.
  • Relevant mentorship qualification or willingness to complete.
Desirable criteria
  • Evidence of continued professional development / CPD modules to degree level.

Knowledge and Expertise

Essential criteria
  • High level of clinical expertise
  • Understanding and appreciation of national policy, health / social care issues.
  • Knowledge and understanding of physical health needs and comorbidity
  • Ability to manage risk.
  • A willingness to undertake further training.
  • An in depth knowledge of primary care mental health and IAPT.
Desirable criteria
  • Experience of working within a Primary Care setting.
  • Experience of working within a Community Mental Health Team
  • Experience of undertaking surveys or audits necessary to own work.

Experience

Essential criteria
  • A minimum of 2 years post registration experience.
  • Experience of working in a multi-disciplinary and multi-agency environment.
  • Experience of writing comprehensive care plans and completing risk assessments.
  • Supervision
  • Substantial experience of managing a busy caseload
Desirable criteria
  • Experience of following policies and providing comments to proposed changes to practice / procedures in own work area
  • Experience of conducting professional / clinical supervision.
