We are seeking an enthusiastic, proactive advanced clinical practitioner with experience working in the community. 

The role involves working within a community nursing team, supporting the team to offer a high level of care to patients in the place they call home.

The post holder will be part of the Urgent Community Response service, visiting patients who are acutely unwell and assessing their clinical and functional needs to try to reduce unnecessary hospital admissions. They will also be involved in the development of the virtual wards, working closely with hospital colleagues.

Hours per week: 30.00

The post holder will use their skills within the four pillars of Advanced Practice  to deliver a very high level of care to complex patients in the community. 

They will assess patients clinically, provide clinical leadership and service improvement and development through research and education.

Qualifications

  • Hold a current professional registration such as NMC (Nursing and Midwifery Council) or HCPC (Health and Care Professional Council)
  • First Level degree or relevant qualifications
  • MSc (Master of Science) Advanced Practice or equivalent Clinical Masters (to include as a minimum clinical examination/diagnostics, independent Non-Medical Prescribing, Leadership, and research)
  • Recognised teaching/ assessing qualification
  • Evidence of professional/clinical knowledge managing acutely unwell patients in the community care setting

Knowledge

  • Advanced knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
  • Clear and demonstrable evidence of advanced, autonomous practice in the management of patients / service users
  • Knowledge and understanding of Clinical Governance and how it applies to role
  • Knowledge of mental health assessment and safeguarding vulnerable adults
  • Knowledge of infection control
  • Professional awareness and knowledge of the National agenda/developments and innovations in Advanced clinical practice and specialist area
  • Understanding of Clinical Governance
  • Risk management and root cause analysis

Skills

  • Advanced, person-centred clinical assessment for the health and well-being of patients with complex needs
  • Physical clinical examination skills
  • Performs clinical procedures appropriate to the patients, needs including further invasive testing and treatments requiring highly developed skills and precision

  • Excellent communication skills, written and verbal
  • Able to sustain and develop relationships across professional boundaries
  • Computer literacy
  • Ability to work logically and complete tasks
  • Ability to work independently
  • Advanced clinical assessment and diagnostic reasoning.

Experience

  • Proven experience of working as an independent practitioner
  • Understanding of frailty and end of life processes.
