Job overview

 There has been an exciting opportunity become available for a passionate and proactive individual who wants to develop their career within the Community Learning Disability Team in East Cheshire.

Under the guidance of a qualified practitioner, you will implement care plans and provide support for service users within the community learning disability setting, and report on the service users wellbeing. 

The post holder will work flexibly in a range of settings, and in partnership with other agencies, carers/family members and the wider multi-disciplinary team.

There will be opportunities for continued professional development, including training around Positive Behaviour Support (PBS) and Trauma informed care.

The team works across two bases, Macclesfield and Crewe.

Main duties of the job

Working as part of the Community Team, you will undertake short term, focused work with the aim of preventing a person going into crisis and being admitted to hospital.

You will provide advice and training to the wider community team, families and care providers to ensure support plans are in place, and work with other health agencies to support successful discharges from hospital into the community.

There will be an expectation that you will travel to work with people across the footprint of the Trust and out of area placements as well.

Qualifications

Essential criteria
  • Appropriate Learning Disability / mental health related NVQ level 2
  • To demonstrate a range of work procedures and practices base level of theoretical knowledge
Desirable criteria
  • NVQ level 3 in LD / Mental health

Knowledge and Expertise

Essential criteria
  • Clear understanding of working within a Person Centred framework
  • Understanding of positive behavioural support and the importance of consistency in the delivery of support and care
  • Knowledge and management of clients with LD and other associated health needs
  • Record keeping and report writing skills, including IT skills to manage electronic patient notes
  • Ability to demonstrate how to Implement clinical care/care plans Ability to undertake observations and record these accurately
  • Good computer / IT skills
Desirable criteria
  • Special interests related to one or more of the teams priority areas (challenging behaviour, autism, active support, communication)

Experience

Essential criteria
  • Experience of working with people with learning disabilities in a range of settings
  • Experience working with people who have challenging behaviour
  • Experience of supporting people with a learning disability in planning and implementing activity programmes
Desirable criteria
  • Working with a community team or other MDT.
  • Experience of demonstrating modelling activities to less experienced support workers/carers.
