Job overview

An exciting opportunity has arisen for an enthusiastic and forward-thinking Band 6 Nurse to join the Transfer of Care Hub (Integrated discharge Team) Fixed Term or Secondment until end of March 2025. 

The successful candidate will promote the Discharge to Assess model by facilitating safe and timely discharges for patients from Macclesfield District General Hospital, supporting the Acute Medical Ward and and as necessary for patients in community Pathway 2 beds transfer to pathway 3. 

The successful candidate will have a passion for putting the patient at the centre and have experience of working collaboratively with multi-professional partners.

Hours per week: 37.5 hours (would also consider part time)

This position is a fixed term post until 31-Mar-2025.

Secondments also considered - if applying internally please discuss with your line manager to check they would be able to release you for secondment before applying. 

 

Main duties of the job

To coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital supporting proactive reduction in delayed transfer of care.

To provide assessments for discharge pathways 1-3.

Working proactively within an integrated health and social care team, to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.

Qualifications

Essential criteria
  • Current first level registration in General Adult/Mental Health Nursing. Degree in Physiotherapy or Occupational therapy with a experience
  • ENB 998 / Clinical Educator Training or equivalent
Desirable criteria
  • 1st Degree level education (or demonstrates ability to study at this level)

Experience

Essential criteria
  • Multi-agency working
  • Work within a multi-disciplinary setting
  • Discharge planning
Desirable criteria
  • Experience of completing continuing healthcare assessments
  • Experience of single assessment process
  • Experience of managing a group of staff to deliver patient care
  • Proactive approach to change and ability to adapt change

  • Proven record of well- developed assessment skills Understanding and knowledge of the Mental Capacity Act and DOLS
