This is an exciting opportunity for an enthusiastic, inspiring and reliable Community Health Care Assistant, who is willing to support and deliver a range of high quality nursing and phlebotomy services to patients in their own homes who are registered with GP Practices within the Neston and Willaston  catchment area.

A commitment to support our team in delivering high quality services is essential for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, trustworthy and approachable.

We are a friendly, compassionate and highly motivated team that work hard to deliver the best outcomes possible for our patients and are seeking someone with similar values and behaviours to join us. 

Previous experience in this role would be an advantage. Travel is an integral part of this post.

'CWP has adopted the Living Wage.  Where the standard Agenda for Change hourly rate is below Living Wage, pay will be supplemented to this level.'

Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital.

As the role involves visiting patients in their own environment, access to transport for work purposes is essential.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.

Qualifications

  • Appropriate related qualification - or willingness to complete an apprenticeship appropriate to the Clinical Support Worker role
  • Completion of or working towards an apprenticeship and gaining qualification equivalent to level 3 or above in a subject relevant to the role of Healthcare Assistant
  • To demonstrate a range of work procedures and practices and to exhibit your knowledge and abilities
  • Completion of or commitment to complete the Care Certificate (CWP In-house 2- day training to be completed within 3 months of start date)

Knowledge and Expertise

  • Understanding of need for confidentiality; empathy and sensitivity

  • Basic risk assessments and record keeping

Experience

  • The ability to demonstrate good quality care, whilst adhering to Trust policies and procedures, and working towards completing Care Certificate
  • Experience of working collaboratively i.e., with colleagues, wider teams, service users etc
  • Supporting others in a care setting, whether this is experience in a professional or lived experience (friend, family, voluntary etc.)
