Advanced Clinical Practitioner - Frailty

Working alongside our existing Advanced Practitioners and experienced MDT,  the post-holder would be responsible for providing a service across the acute frailty pathway.   This role requires excellent organisational and interpersonal skills, alongside sound clinical knowledge, relevant experience and the ability to work at pace.

The acute frailty pathway gives advanced clinical practice high priority.  Supporting people living with frailty to self-care and maintain what matters to them most are our key functions.   We provide a truly integrated pathway, collaborating with our partners in primary care and local authority to improve patient care and communication across the whole system.   The successful post holder will be expected to use their qualifications, skills and expertise to continue support and development within these areas.

This is a rotational system wide post, where you will be responsible for providing specialist clinical support to the whole frailty pathway;  ED inreach, the acute inpatient bed base,  Acute Frailty Same Day Emergency Care , and supporting our Acute Frailty Virtual Ward.    Although the role is predominantly clinical, there will approximately 25% of the role dedicated to managing and supervising the duties of junior members of the MDT.

This role is an exciting opportunity for you to be part of a new system wide model of care for our population living with frailty.   You will be assessing patients with complex needs, undertaking physical examinations using appropriate use of investigations in order to reach a diagnosis.   As part of the team you will support in developing and contributing to the improvement of services.  We are looking for someone who can demonstrate evidence of autonomy, clinical judgement who has excellent communication and decision making skills.

The WHH Frailty Hub pathway works on a “Home First” philosophy – with a focus, where appropriate, to return patients to their usual place of residence or onward destination, on the same day. As part of the specialist multidisciplinary team you will be essential in delivering a Comprehensive Geriatric Assessment, coupled with access to timely diagnostics, coordinated from a dedicated purpose-built Frailty Assessment Unit. The outcome of this assessment will result in the development of a Care and Support Plan, developed collaboratively between the specialist teams with the patient and their carer’s. This CSP will focus on the person’s assets and strengths, as well as identifying areas where they may need support from health and social care professionals.

  • Minimum 5 years post registration experience
  • Broad range of clinical experience including recent experience of acute medicine or community services
