An exciting opportunity has arisen for a Band 4 Cancer MDT Coordinator at Warrington and Halton Teaching Hospitals NHS Foundation Trust. The post will be working with the Colorectal Team
Working with our existing MDT Co-ordinators we are looking to add to the team to provide full support in all aspects of this role and to provide MDT Co-ordinator cover across all tumour groups.
Within this you will be responsible for data collection and input, facilitating MDT meetings and tracking patients referred to the Trust with a suspected and confirmed cancer diagnosis.
You will work closely with the Clinical Nurse Specialists and the Multi-Disciplinary Teams within the specialties as well as the rest of the Cancer Services Team.
The Cancer Management team is committed to the personal development of team members and there will be opportunities for development within this role.
Enthusiasm, commitment and flexibility are required for this role along with good customer service and excellent organisational and communication skills. Ability to work under pressure in a fast paced environment with excellent attention to detail is absolutely essential.
Main duties of the job
· The post holder is responsible for the facilitation of Multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured.
· To ensure that patients are discussed according to evidence-based protocols and in line with the MDT operational policies.
· To provide administrative support to the multi-disciplinary team meetings preparing and maintaining all related documentation and information.
· To ensure patient treatment plans and multi-disciplinary team decisions are recorded onto designated proformas, transferred into medical notes and are updated on the relevant electronic systems.
· To input outcome data into local systems, in a timely manner, during the MDT or immediately after the MDT, ensuring data accuracy at all times.
· To be involved in the recording of data that supports the collection of the Cancer Outcomes Data Set, Cancer Waiting Times, audit data sets for cancer services.
· Responsible for tracking patients through their pathways to support the delivery of performance against all cancer access targets.
· Provide support documentation of any breaches through investigating the causes.

Detailed job description and main responsibilities
MDT Meeting Coordination
· Liaise with the Clinical Team to ensure that all patients who require discussion at the MDT Meeting are added to the agenda
· Prepare the agenda for the MDT Meeting and ensure that is it sent out to participants within the agreed timescales
· Ensure that all required documentation such as casenotes, imaging and pathology are present for the meeting so that discussion can go ahead as planned.
· Maintain an attendance register for each meeting
· Record the outcome of the MDT discussion for each patient and agree as correct with the clinical team. Ensure that this outcome is documented on the Somerset system. Circulate minutes from the meeting within agreed timescales
· Proactively query information required for National Clinical Datasets and COSD from Clinicians during the meeting
· Proactively prompt the Clinical Team about patient pathway information.
· Manage the video link up when required and troubleshoot any issues that occur during the meeting.
· To assist other members of the team and provide meeting cover in times of absence within the team.
Tracking of Cancer Patients
· To track patients from urgent fast track referral throughout their pathway of care in line with all cancer access targets. This includes 62 day (Classic/Screening/Upgrade) and 31 day (First and Subsequent) and ensure that they are seen within appropriate timescales at each point in the pathway
· Ensure that Somerset is updated appropriately with relevant tracking comments
· To identify potential breaches of any pathway and escalate according to the Trust escalation policy
· To attend and participate in the weekly PTL meeting
· To ensure that the appropriate information is sent to other Providers at the point of transfer of care in a patient pathway and that this is recorded accordingly on Somerset.
· Producing patient pathway information to support the RCA process for any patients who breach the 62 day target.
· To ensure that tracking requirements are met during times of absence in the team.
- To be a point of contact for internal personnel and external organisations regarding Cancer data, resolving queries independently or referring on to the most appropriate member of staff as required.
Data Collection
· Collect all data pertaining to Cancer Waiting Times targets, COSD and National Audits ensuring that this is timely and that excellent data quality is maintained as directed by the Cancer Data Manager
· Liaise with the Clinical Teams to ensure that data is correct.
· Ensure that data is validated and corrected where necessary.
- Take action to obtain or correct missing or contradictory data whilst maintaining confidentiality.
- Responsible for planning and organising own work schedule in order to meet specific deadlines.

- • Experience of working in an NHS administrative or clerical role. • Experience of using patient information systems. • Knowledge and experience of patient pathways. • Knowledge of Cancer Waiting Times targets. • Computer literate and experience of using the Microsoft Office Suite. • Experience of working to tight deadlines and across organisational boundaries
