An exciting opportunity has arisen for General practitioner to join our team at Mid Cheshire Hospitals NHS Foundation Trust and make a meaningful impact on patient care. We are currently inviting applications for General Practitioners to provide medical cover at our community beds currently Elmhurst care home (Winsford), Station House care home (Crewe), Telford care home (Crewe), Twyford House care home (Alsager). Successful candidates will provide essential support for the Division of Diagnostics and Clinical Support Services.

 As a General Practitioner in this role, you will play a pivotal part in the GP-led step-down and rehabilitation wards. Your responsibilities will involve supporting patients in their journey towards improved function and facilitating their discharge planning, whether it be to return home or to another place of care.

Main duties of the job

• Collaborate with an exceptional team of multidisciplinary staff to facilitate the safe and timely admission, rehabilitation, and discharge of patients in GP step-down wards.

• Take primary responsibility for the assessment, treatment, and management of patients on the ward, working closely with the wider multidisciplinary team to ensure comprehensive care.

• Update patients' treatment and management plans on a daily basis to reflect their evolving clinical needs and rehabilitation goals.

• Maintain precise and contemporaneous electronic medical records using EMIS software, ensuring compliance with regulatory standards.

• Respond promptly and appropriately to any changes in patients' clinical conditions, adhering to the Trust's escalation policy to ensure timely interventions and patient safety.

• Foster a collaborative and supportive environment within the team, contributing to the overall success of patient care delivery and outcomes aimed at promoting safe flow of patients discharging back to their own home setting.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

Detailed job description and main responsibilities

Elmhurst Intermediate Care Centre is a highly regarded facility, located in Winsford, praised by the Care Quality Commission (CQC) for its outstanding contribution to the rehabilitation needs of the local population. The centre operates as a 30-bedded unit spread across two floors, featuring individual rooms for patients. Led by a team of dedicated nurses and therapists, Elmhurst provides comprehensive rehabilitation services with the support of General Practitioners (GPs) and Advanced Clinical Practitioners (ACPs). Alongside this facility we have 12 beds at Twyford House care home (Alsager), 10 beds at Station House care home (Crewe) and 10 beds at Telford care home (Crewe).

Key Responsibilities:

1.    Patient Assessment and Management:

Conduct thorough assessments of patients admitted ensuring suitability for rehabilitation.

Develop and implement tailored treatment plans in collaboration with the multidisciplinary team, addressing the individual rehabilitation goals of each patient.    

Monitor patients' progress closely, adjusting treatment plans as necessary to optimize outcomes and functional independence.

2.    Collaboration and Communication:

Work closely with nurses, therapists, and Advanced Clinical Practitioners (ACPs) to coordinate and deliver holistic care to patient.

Maintain open and effective communication with patients, their families, and caregivers, providing updates on treatment progress and addressing any concerns or questions.

3.    Medical Oversight and Support:

Provide medical oversight and support to patients at Elmhurst Intermediate Care Centre, ensuring that they remain medically stable throughout their rehabilitation journey.

Collaborate with the Placement of Care Hub at Leighton Hospital to ensure that patients are medically fit for admission to community and further support flow of patients for discharge to baseline home setting for further rehab.

4.    Quality Assurance and Compliance:

Adhere to all relevant policies, procedures, and regulatory requirements to maintain high standards of care and patient safety.

Participate in quality improvement initiatives and clinical audits to enhance the delivery of services at Elmhurst Intermediate Care Centre.

 Qualifications and Requirements:

 Medical degree from an accredited institution

Full registration with the General Medical Council (GMC) and on the GP Register

Ideally have experience working as a General Practitioner in a similar setting, preferably in rehabilitation or intermediate care

Strong clinical assessment and decision-making skills

Excellent communication and interpersonal abilities

Essential criteria
  • Full GMC Registration
  • Hold appropriate medical qualification to practice medicine in the UK
  • MB ChB or equivalent certificate of prescribed/equivalent training in General Practice
  • Inclusion on the Performers List with no restrictions to practice
Desirable criteria
  • Current resuscitation certificate

Experience

Essential criteria
  • Experience of medical, surgical, elderly care, rehabilitation and mental health.
Desirable criteria
  • Over six months’ experience of working independently in General Practice

Knowledge

Essential criteria
  • Understanding of the principles of evidence-based medicine and rational prescribing
  • Ability to take a risk-based approach to support discharge planning
Desirable criteria
  • Awareness of NHS plans and ongoing developments in healthcare.
