Clinical Support Worker

Job overview

The successful applicant will be responsible for working alongside the SWMS clinicians and supporting patients at specific community venues across the areas we cover; they will also be required to provide cover at other venues. Please note, this may, occasionally, include evening cover at The James Cook Hospital or Friarage Hospital, Northallerton.

A day in the life of a Clinical Support Worker will include quick weight checks where they will be responsible for taking a weight of a service user and discussing this, along with any questions the service user may have. They will also run longer review appointments covering discussion of bariatric surgery and discussions about implementing advice given by the clinical team into the patients lifestyle. 

The successful applicant will have previous experience of working in the NHS, and possess good written and verbal communication skills and be able to demonstrate excellent team working skills. 

Main duties of the job

Key Tasks

  1. Work the SWMS Multidisciplinary team and Administration team to deliver an effective service for our clients.
  2. To help people in the Hartlepool, Teesside, Redcar & Cleveland, North Yorkshire, Sunderland, Durham and Darlington, and South Tyneside areas who are referred to the SWMS to lose weight and maintain weight loss.
  3. To interact with clients on both an individual and group situation.
  4. To feedback relevant information to the SMWS Multidisciplinary team and Administration team as appropriate.
  5. To support the SWMS Administration team with regular duties and cover to ensure an effective service for our clients and clinicians.
  6. To work closely with Tier 2 and Tier 4 weight management services.
  7. To contact other stakeholders as required to given or to request more information e.g. GP surgeries.
  8. Promote the equality and value the diversity of individuals
  9. Build up and maintain knowledge of contacts within the community.

Enable individuals to change their behaviour to improve their health and weight

  1. Help individuals identify how their behaviour and context might affect their health and weight
  2. Help individuals develop personal goals and assist them to carry out advice from the SWMS Multidisciplinary team.
  3. Support individuals in maintaining their behaviour change
  4. Help individuals to access and use local services as appropriate.
