The Discharge Suite provides a safe and comfortable environment for patients to wait for medications, family members or transport ahead of transfer to their discharge destination. Optimal utilisation of the discharge Suite is essential to ensure timely availability of beds and improve patient flow through the hospital.

 

 

Main duties of the job

The main duties of the role are: 

  • Maintain the effective day to day running of the department
  • Ensure that the criteria for referral to the Discharge Suite are followed and maintained
  • Ensure patients receive any medications they are due prior to discharge
  • Check issued medication against prescription for all patients, noting that the patient may have been waiting for supplies to be sent from pharmacy as well as having some supplies from the ward. Liaise with pharmacy and ward medical staff if there are any errors.
  • Ensure all patients leave hospital with the correct medical information, equipment, dressings, dietary supplements and medication, ensuring the patients know how to obtain further supplies.
  • Escalate any issues with care packages as appropriate.
  • Continue with pressure area care and falls risk assessment
  • Escalate any medical issues to ward staff
