Assistant Practitioner / Nursing Associate
Band 4

There is the opportunity to recruit an Assistant Practitioners or Nursing Associates to work with the Nursing and Healthcare team on the Discharge Lounge.
The Discharge areas comprise of a busy, proactive 9 bedded unit , and a 10 chaired Ambulatory discharge lounge that cares for patients that are going if in the Ambulatory Discharge Lounge. We work closely will all other wards to ensure that we create flow through the Hospital.
We are looking for a pro-active and willing Band 4 AP/NA to support the delivery of care and safe discharges to our patients.
As a Band 4 on the area you will be supported to nurse a cohort of patients and be responsible for their care, leading on their care and joining ward rounds to ensure that there is a clear plan of discharging patients safely.
Part time - 27.5 hours per week available.

Assist with and undertake patient assessment (under supervision and
after appropriate delegation from the registered practitioner).
Ensure that patient care have been met
Maintain patients’ personal and oral hygiene, e.g., help with
dressing/ undressing, giving bed baths, mouth, and skin care,
assisting with pressure area care, maintaining privacy and dignity.
Assist in providing conditions to meet the patient’s need for rest and
sleep.
Provide and promote health education; support and encourage the
patient to meet their own health and wellbeing needs.
Maintain and improve the patient’s mobility using appropriate
equipment and handling techniques.
Assist the patient to be as comfortable as possible, i.e., help with
moving and positioning the patient using appropriate equipment.
Assist with checking pain levels and requesting pain relieving
medication on patient’s behalf.
Ensure that safe discharge happens for patients.
