Discharge Progress Coordinator

Job overview
We are looking for a part-time Discharge Co-ordinator to support our two wards; Wheal Coates, and Gastroenterology and Liver Unit, alongside our Endoscopy Unit for two days a week. The postholder will be busy with discharging administrative duties supporting the Ward Leader and wider clinical team associated with being an inpatient discharge hub for hospital.
You will work in this new model of care alongside an integrated multi-professional team of rostered practice educators, registered nurses, occupational therapist, physiotherapists, dietitians, speech therapists, podiatrists and of course health care support workers.

Main duties of the job
We are looking for brilliant colleagues who:
Have proven administrative experience, particularly around discharge and the discharge to assess pathways and working as part of a multi professional team.
Are committed to exceptional care of older people transitioning from hospital to home.
Have digital proficiency – Microsoft applications and keyboard skills.
Have a good standard of English and maths education, equivalent to GCSEs.
Are extremely well organised with brilliant communication skills.
Are able to prioritise demanding workloads to meet deadlines.

Qualifications
Essential criteria
- Relevant IT skills to include, E-mail, Excel, Window Word (equivalent to RSA 3, European Driving Licence)
- NVQ Level 3 in Care/Business Administration or equivalent qualification or relevant practical experience
Desirable criteria
- Comprehensive knowledge of issues relating to discharge planning
- Some knowledge of infection control
Experience
Essential criteria
- Worked within a Health or Social Care Setting
- Multi-disciplinary working and ability to participate in interdisciplinary decision making
Desirable criteria
- Working with PAS, MAXIMS, and Swiftplus
- Experience of ward based work
- Experience of the discharging of patients to further health care settings
- Bed management experience
