Job overview

This is an exciting opportunity for a registered nurse wanting to further progress their interest in diabetes. The job requires working autonomously, with access to the Diabetes Specialist Team, in providing evidence based and high-quality diabetes care and education in both the in-patient and out-patient setting.

To assess, plan, implement & evaluate individualised programmes of evidence-based care providing specialist Diabetes advice to patients, carers/relatives, and the multidisciplinary team.  

To contribute to the organisation and delivery of ongoing education and training for other healthcare professionals.  In conjunction with the Diabetes Multidisciplinary team assist in the development, implementation and adoption of local diabetes care policies and guidelines. Take part in local and national research and audit. 

Main duties of the job

In conjunction with the Diabetes Multidisciplinary team develop your knowledge, skills and experience of Diabetes including complications, medication and with the promotion of self-management as a corner stone of diabetes care. Contributing to the assessment process and the creation of the most effective and appropriate care programme strategy as agreed with the person with diabetes and their families/ carers while recognising individual diversity.

Have continuing responsibility for the provision of high-quality diabetes specialist nursing care across University Hospitals of Morecambe Bay to include inpatient and outpatient setting.

Undertake nurse led clinics (face to face, telephone or virtual) autonomously with access to remote support. On occasion an assessment and support in the persons home environment may be required. 

Working for our organisation

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Education and Qualifications

Essential criteria
  • 1st Level Registered Nurse
  • Post registration qualification in Diabetes or willingness to undertake
  • Evidence of continued professional development
Desirable criteria
  • Educated to Master's level
  • Non- medical prescriber (this is a qualification to work towards)

Experience

Essential criteria
  • Recent management/leadership experience
  • Current knowledge, experience and competence in the clinical management of patients with Diabetes
  • Setting and monitoring of standards
  • Ability to develop expert practice
  • Demonstrate change management
  • Experience of establishing effective communication
Desirable criteria
  • Experience to presenting to a multidisciplinary audience
