Job overview

Your role will  involve clinic/group work, to provide thorough, holistic, patient centred assessments and plans of care, working closely with our own MDT (Physio, support staff, ACP and Nurse team mates) as well as the wider MDT (general practice, community services, third sector and Hospital colleagues).

Our caseload involves those referred for Pulmonary rehabilitation (inc. Home-PR),

We are looking to recruit someone who is caring, enthusiastic, hard working, with a passion for Respiratory, delivering high quality community patient care and patient experience,  and demonstrate excellent teamwork.

Experience of Community working would be favourable, however, this post would also suit an experienced hospital or general practice Nurse. We expect you to demonstrable an interest in, and willingness to develop further in the field of Chronic Respiratory Disease. Please feel free to contact our team to discuss the role further or arrange opportunities to shadow the team to get a feel for community respiratory team working.

The Community Respiratory service is undergoing transformational developments as part of the Morecambe Bay Respiratory Network. As such, you will be involved in our various Quality Service Improvement Redesign projects, to help improve the quality and experience of our services for our patients.

Main duties of the job

The posts will support the Community Respiratory Service to support the provision of the Pulmonary Rehabilitation and our domiciliary Nursing caseload.

Working as a Community Respiratory Nurse as part of the  Community Respiratory Service across the North Lancs area, involves supporting those who are living with and struggling to manage their chronic lung disease.

Specific duties are outlined in the job description.

Working for our organisation

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life-threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
