Job overview

We have a fantastic opportunity for a highly-motivated and enthusiastic Care Co-ordinator to join the community teams within  Derby City.

Key aspects of the role will involve identifying and supporting patients who require additional support within the Community in line with the Ageing Well agenda, referring and signposting patients within the Health, Social and Voluntary Sectors.

The role will also include working closely with the aligned GP Surgery to  coordinate meetings & Patient MDT meetings with Health and Social Care colleagues.

The successful applicants will have excellent communication skills, be able to work independently and be confident using IT systems.

This is a really exciting and challenging role for the right person; someone who is committed, has excellent IT and time management skills and really wants to make a difference to a high-functioning health care team.

Travel will be required across DCHS sites in Derby City within your working day.

Main duties of the job

Key aspects of the role will involve:

Identifying patients who require additional support within the community, using IT systems such as SystmOne reporting.

Co-ordinating monthly CST (Meetings and individual patient MDT meetings with external parties, such as Social Services and Community Mental Health teams and members of the Integrated team, to support complex patients.

Liaising with health care professionals, including GP, Advanced Specialist Practitioner/Community Specialist Practitioner, District Nurse, Therapies and other members of the health care team.

Referring patients to the voluntary sector as well as the health sector for additional support in and around their home and making telephone contact with patients; acting as liaison between the patient, health professionals and the social sector.

The Care Co-ordinator will have a holistic approach towards supporting patients within the community, helping them live well at home, working alongside an aligned GP surgery.

Qualifications and Education

Essential criteria
  • ECDL or equivalent
  • NVQ level 3 in Business or equivalent
Desirable criteria
  • Long Term Conditions training

Experience and Knowledge

Essential criteria
  • Able to demonstrate a clear understanding of working with confidential information
  • Experience of working with healthcare professionals.
  • Experience or knowledge of Social Services and Voluntary Sector
  • Able to demonstrate a clear knowledge of working with key stakeholders
Desirable criteria
  • Experience of co-ordinating patient care
  • Previous experience in a primary care setting/working in a GP practice

Skills and Attributes

Essential criteria
  • Highly proficient in the use of Microsoft Office, databases, internet and Outlook/emails
  • Excellent time management, prioritisation and managing own workload skills
  • Excellent organisational and administration skills
  • Experience in managing meetings
