Job overview

We are seeking to appoint a dedicated and motivated Health Visitors to work within the South Derbyshire locality.

The South Derbyshire 0-19 Team are a Gold accredited locality for their work with Families and Communities.

The right candidate will hold the SCPHN qualification in Health Visiting and the V100 and be an innovative and dynamic team player who is looking for a new challenge.

Main duties of the job

To  deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.

  • To provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme.
  • To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
  • To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
  • To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
  • To manage a team of health visiting support staff.

Education, Quals & Training

Essential criteria
  • RN or RM
  • HV Qualification – Degree, Diploma or Cert
  • Nurse Prescribing
Desirable criteria
  • Specialist Community Public Health Nurse Post registration study
  • Mentorship qualification

Experience and Knowledge

Essential criteria
  • Knowledge of national + local priorities
  • Evidence based Health improvement initiatives/public health activites
  • Working in partnership with clients and carers
  • Experience of mentoring students in practice
Desirable criteria
  • To have actively practised as a HV within the last 5 years or recently undertaken SCPHN training
