Are you keen on learning new skills and becoming an autonomous Nursing Associate who can provide patient-centred care in the patient's home?

We are a professional, skilled  and friendly District Nursing  teams who work safely with our patients in the North East Derbyshire area.  We want to make a difference to our patients lives by providing evidence-based nursing care. 

We provide training and education  for the successful candidate and are a team that likes to learn together with regular teaching sessions and information sharing to ensure our standards of nursing are high.

Prif ddyletswyddau'r swydd

Provide high quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.

Implement/refer to the relevant services as appropriate. 

Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response, we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
