The Discharge Lounge team
Contributes to the delivery of outstanding patient care by:
Ensuring safe, effective and timely discharge from the trust to another place of care or to their own place of residence.
Communicating openly with patients, their families and other health/social care professionals.
Providing a comfortable and relaxing environment for patients awaiting discharge, where ongoing care needs can be met with support from dedicated members of staff.
Facilitating patient flow through the hospital.
Monitoring discharge progress and escalating any concerns timely to prevent delays/prolonged hospital stays.
Main duties of the job
To provide excellence in nursing care to all patients utilising the discharge lounge.
With supervision and support from the Ward Leader, to take charge of the day-to-day operation of the discharge lounge.
To provide professional nursing leadership, guidance and clinical advice to staff members.
To assist in the effective operational management of the ward and flow of patients within the hospital, concentrating on the provision of quality care for patients.

Qualifications and Training

- 1st Level RGN
- Demonstrable post registration professional Development
- Diploma/Degree in Nursing
