To actively monitor the multidisciplinary management and discharge plan for all patients on designated ward(s) and take action to expedite the process, avoid delays and thereby improve the patients’ experience

A critical success factor in this role is reducing the length of stay and ensuring that the discharge is planned and executed for all patients. For some patients this will mean reducing stay by hours.
