Job overview

We have an exciting opportunity for a Community Therapy Support Worker to join our Therapy Service in Chesterfield and Dronfield locality within our core Therapy services!

The role will require weekend working on a rota basis within our rapid response therapy service following an initial; orientation and probationary period.

The role requires day-to-day delivery of therapy in the community, working alongside the Physiotherapists, Occupational Therapists and Advanced Therapy Support Workers in patients own homes and care homes. 

You will be delivering rehabilitation plans established by the team to work on achieving the goals set. 

Main duties of the job

Our therapy services operate  between the hours of 08:00am and 18.30pm seven days a week.

You will be required to travel to your visits and be able to operate a laptop to write notes onto SystmOne. You will need to exhibit excellent team working and communication to be able to assist in caseload management and patient flow.

Education, Qualifications and Training

Essential criteria
  • NVQ Level III in care / BTEC Health & Social Care or equivalent
Desirable criteria
  • Additional training in:- Rehabilitation, Palliative Care, Care of the Older person

Experience and Knowledge Required

Essential criteria
  • Previous experience of team working in a health/social care setting
  • Understanding of factors that influence health and wellbeing
  • Understanding of the implications for, and responsibilities of the post for Risk Management and Safeguarding

Other

Essential criteria
  • Able to work flexibly to meet service demands
  • Access to and ability to use transport as travel between and across care settings will be required
  • Able to work across daytime shifts and work weekends and bank holidays
