We have a fantastic opportunity for a Senior Community Nurse to join Our Rapid Response Community Nursing Team within the Chesterfield and North East Derbyshire Locality.
This is an ideal opportunity for a registered nurse with varied community experience who wants to develop their leadership and Triage management skills in a supportive environment.
Main duties of the job
The successful applicant would require community nursing experience, knowledge of caseload management with a high standard of community nursing knowledge and clinical skills. Leadership experience desirable.
You will be provided with support tailored to your needs to drive forward high quality care and team work; including a thorough induction programme, regular one to ones and peer supervision.
Provide effective evidenced based care to a defined population/Primary Care Network. This is achieved through effective delegation, case load management, monitoring of care standards, leadership and the performance/ line management of team members.
To assess, plan, implement and evaluate individualised patient care, utilising their professional judgement at all times.
Provide clinical leadership and line management to less experienced Nurses, and other members of the multidisciplinary team in the delivery of high quality care.
We work across three bases with the locality: Clay Cross Hospital, Walton Hospital and the The Arc. There is a expectation to be able to travel and be able to work across each base.
To contribute to the continuous improvement of community nursing within their Place, linked to the 10 year NHS plan.

Education, Qualifications and Training
Essential criteria
- Registered nurse with current NMC registration
- Relevant Nursing degree or equivalent relevant experience
- Student mentorship
- Have undertaken or be prepared to undertake a leadership development programme
Desirable criteria
- District Nurse specialist qualification
Experience & knowledge
Essential criteria
- Post-registration experience
- Case management/ward management experience
- Community experience
- Proven clinical expertise, knowledge and experience of implementing service improvement
- Knowledge of Public Health agenda
Desirable criteria
- Experience in a leadership role formally or informally
Skills and Attributes
Essential criteria
- Commitment to the development of clinical practice of others
- Excellent communication skills both verbal and written
- Access to transport is essential for this post, as you will be expected to travel around various areas of Derbyshire in order to carry out required duties
Desirable criteria
- Proven managerial skills and experience of change management
