To manage referrals into our service and provide evidence-based care to support patients in their own home. 

This includes  working alongside general practice/therapy/district nursing colleagues  and  services within the acute  hospitals to prevent hospital admission and support discharge home.

To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.

To provide advice and support to junior staff in the delivery of high quality care and actively support the community nursing sister/charge nurse in the management of the health care environment.

Our Rapid Response team is developing and growing to support the District Nursing teams across Derby City to provide a high quality service to our patients. 

We are a professional, friendly, and enthusiastic team who work closely with our therapy colleagues to provide a holistic integrated  service that enables and educates our patients to remain independent in their own homes, whenever possible, to prevent hospital admission and support a safe discharge home.

This is increasingly important as care for patients become more community-focused.

Our purpose in Derbyshire is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
