Pharmacy professional–Community Pharmacy Integration Programme Manager

Following the Derby and Derbyshire ICB staff restructure an exciting new senior role has become available within the Pharmacy Directorate Team, which is part of the Medical Directorate.
Supporting the Community Pharmacy clinical lead, you will be providing project management support to implement community pharmacy integration initiatives, working with the wider healthcare system to integrate community pharmacy services.
This is a 1 year fixed term or secondment post.

We are seeking an experienced and talented Pharmacy professional with significant project management experience to plan the mobilisation and implementation of community pharmacy integration initiatives, building and maintaining stakeholder networks. The post holder will need to be self-motivated, work independently and have good team working skills.
You will support the Community Pharmacy Clinical lead in delivering high quality, safe and effective care across care pathways within Joined Up Care Derbyshire whilst delivering Pharmaceutical Population Health Management.
Support the strategic development of community pharmacy integration and services. There is a changing landscape in the community pharmacy sector and with all graduates from 2026 as independent prescribers lends itself to a sector that will be more clinical services focused. With this and other current initiatives including Pharmacy First and community pharmacy independent pathfinder allows a greater understanding of how clinical services provided in this sector support our wider population health initiatives.

The postholder will be responsible for mobilisation and project management of a range of community pharmacy integration fund services. This is an exciting time as community pharmacy is a critical part of primary care. Working across teams within the ICB and other stakeholders to implement these services. Working alongside other ICB to develop local place and PCN pharmacy networks to include community pharmacy.
Work with their Community Pharmacy clinical lead (CPCL) to facilitate the successful implementation of relevant commissioned services with local community pharmacies and provide specialist pharmaceutical support, advice and monitoring to commissioning service developments, contracts and procurements including contract development, monitoring and management. Lead on designated functions and work programmes.
Will be responsible for supporting pharmacy professionals within the Pathfinder Programme and within the scope of other community pharmacy clinical pathways if required.
Supporting the safe use of medication and pharmacy interventions as provided by Community Pharmacy integrated services. Working with other ICB and ICS teams involved in medication safety to raise awareness and learning.
Work with the CPCL and antimicrobial lead to ensure Antimicrobial Stewardship is delivered by community pharmacy services, e.g. Pharmacy 1st, in Derbyshire in order to improve the appropriate prescribing of antimicrobials; Provide regular Antimicrobial Prescribing Reports, relevant to community pharmacy, to highlight variation. Liaise with the ICB AMR lead and report to/attend the JUCD AMR committee as required.
