Senior Clinical Pharmacist (Oncology and Haematology)

We are friendly and welcoming team of specialist oncology and haemato-oncology pharmacists who work together well in a supportive environment with dedicated technician support.  The chemotherapy service is expanding, and we are able to offer an exciting opportunity to join our team.  

We work in a bespoke cancer centre which opened in 2016, with excellent working environment facilities for pharmacists.  Come and visit us... we feel confident you will want to apply!   We were awarded the Clinical excellence award from Myeloma UK in 2020 and the pharmacy team of the year from the European Association of Hospital Pharmacy in 2021.   Our service provides leading edge patient focussed clinical pharmacy including working closely with nursing and consultant teams, the prescribing of chemotherapy and independent patient assessments.  No experience is necessary, you will be fully trained in this rewarding clinical speciality.    

We welcome applications from pharmacists who would expect to complete the clinical pharmacist post-graduate diploma in the Summer 2023. We would support a prescribing qualification for pharmacists who are interested in developing their skills into this area.

The role includes contributing to the specialist multidisciplinary team which provides inpatient, day case and outpatient chemotherapy for a wide range of oncology and haemato-oncology cancer sites. This will include both inpatient duties, ward rounds and the direct involvement in the specialist oncology and haemato-oncology clinics.

To be an active member of the oncology and haemato-oncology pharmacy team and work as an integrated part of the ward and clinic based oncology and haemato-oncology  service.  

To oversee the safe management of high-risk systemic anticancer treatments.

We are very committed to the development of pharmacist prescribing and have one of the highest numbers of active pharmacist prescribers in the country.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

  • To provide clinical pharmacy services to wards and departments
  • To be responsible for the clinical pharmacy service to specialist haematology ward patients
  • Attend and contribute to the haematology ward round, and MDT meetings
  • Contribute to the pharmaceutical care of patients admitted for “acute oncology” reasons.
  • To be responsible for the safe management of Systemic Anti-Cancer Therapies (SACT) for patients who are admitted to the wards for any reason.
  • To clinically verify prescriptions for SACT and supportive medication.
  • To prescribe supportive medication where indicated.
  • To give specialist advice and support to medical and nursing staff, and other members of the pharmacy team.
  • To give education and training to nurses and other members of the pharmacy team
  • To give advice to patients and carers
  • To counsel patients and confirm consent prior to initiating SACT treatment
  • To communicate effectively with primary care regarding the care of patients on SACT medicines.
  • To act as a pharmaceutical and prescribing adviser, including the production and of review of medicines related guidelines and pathways
  • To contribute to the safe introduction of new SACT
  • To undertake prescribing within a defined speciality, and as part of wider medicines optimisation initiatives undertaken by the clinical pharmacy service.
  • To undertake clinical pharmacy practice at an advanced level.
  • To monitor compliance and advise on the implementation of National Guidance e.g. NICE,
  • To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required
  • To contribute to the delivery and development of clinical pharmacy services across the Trust
  • Actively participate within the multi-disciplinary and clinical pharmacy team
  • To act as a source of expert advice on prescribing and medicines optimisation to medical and nursing staff within a defined clinical area
  • To participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice, within the context of Quality Governance

Experience

  • At least 18 months post registration experience in an acute hospital pharmacy, with an emphasis on clinical work.
